Westside San Joaquin River Watershed Coalition Application/Sign-Up Sheet For Participation in the Irrigated Lands Regulatory Program The undersigned owner and/or operator (owner/operator) of irrigated farmland located within the geographic boundaries of the Westside San Joaquin River Watershed Coalition (Westside Coalition) understands that the Central Valley Regional Water Quality Control Board (Regional Board) has recently issued a Notice of Applicability to the Westside Coalition as a proper third party for administering the terms and conditions in the Waste Discharge Requirements General Order for Growers in the Westside Coalition that are Members of the Third Party (Order No. R52014-0002) (General Order). The undersigned owner/operator also understands that it is his/her responsibility to know and understand the terms and conditions contained in the General Order, and that failure to comply with the General Order terms and conditions may subject the owner/operator to removal from the Westside Coalition, and potentially could subject the owner/operator to enforcement action taken by the Regional Board. A summary of the General Order requirements has been attached for your convenience. A copy of the General Order and other information about the Westside Coalition can be found at www.westsidesjr.org With these understandings, in order for the Westside Coalition to fulfill its obligations as provided under the General Order, the undersigned owner/operator hereby elects to participate in the Westside Coalition and hereby elects to comply with the requirements set forth in the General Order. Failure of an owner/operator to abide by the requirements of the General Order or to pay membership fees when due, may result in the termination of membership and enforcement action by the Regional Board.

Irrigated Acreage - Acreage that is irrigated and used for commercial purposes. Do not include acreage that is covered under a different Regional Board regulatory program (e.g. Dairy General Order). When calculating and reporting irrigated acreage, you may exclude non-irrigated portions of your total parcel acreage. Non-irrigated portions typically include farm roads, county easements, ponds, streams, residential areas, or shop areas. Operators - Includes lessees, tenant farmers, operating entities, and relatives or other individuals farming on behalf of the owner. If listed, the Operator will be contacted to access the property should the need arise.

SUMMARY OF GENERAL ORDER REQUIREMENTS The following list is a summary of General Order requirements for Coalition Members. This list will likely be replaced by a General Order Summary from the Executive Officer of the Regional Board: 1. Members shall maintain a copy of the General Order at the primary place of business or the Member’s headquarters for its farming operation so as to be available at all times to operations personnel (General Order IV.B.18); it is available for download at www.westsidesjr.org or by request. 2. The Member, or his/her designee, is required to be familiar with the contents of the General Order. (General Order IV.B.18) 3. Members shall provide the Coalition with the phone number(s) of the individual(s) with authority to provide consent to access its facilities. (General Order IV.B.14) 4. Members shall provide the Coalition with any information requested to demonstrate compliance with the General Order. (General Order IV.B.5) 5. Members shall submit all required reports in a complete and timely manner. (General Order IV.B.19) 6. Members shall, at a minimum, implement water quality management practices that meet the following farm management performance standards: (a) minimize waste discharge offsite in surface water, (b) minimize percolation of waste to groundwater, and (c) protect wellheads from surface water intrusion. (General Order IV.B.20) 7. Members shall implement water quality management practices with any water quality management plans prepared by the Westside Coalition and approved by the Regional Board. (General Order IV.B.6) 8. Members shall prepare and implement a farm-specific nitrogen management plan as required by the General Order. (General Order IV.B.8) 9. Members shall implement the applicable management practices, or equivalent practices, identified as protective of groundwater in the Management Practices Evaluation Report. (General Order IV.B.21)
