American Fork High School Wall of Fame To nominate someone for American Fork High School’s Wall of Fame, please write a letter of nomination addressed to Wall of Fame Committee, American Fork High School, 510 North 600 East, American Fork, Utah 84003. To be considered for the 2015-2016 school year, the letter of nomination must be received by April 18, 2016. To qualify as a nominee for AFHS Wall of Fame, a candidate must • • •

have graduated at least 10 years ago Be a graduate of American Fork High School Have made a significant contribution the American Fork Community, the State of Utah, the United States of American, or the world through a high level of achievement in their field, through service, etc.

Name of nominee Your name Nominee’s graduation year Nominee’s involvements in high school Nominee’s contributions to American Fork Community, State of Utah, United States of America, or the world (high level of achievement in field, service, etc.) Specific details and time frame of contribution Any additional information you feel to be appropriate

Please note: Nominations for Wall of Fame will be kept from year to year. Therefore, there is no need to resubmit a name. There is no guarantee that someone will be placed on the Wall of Fame every year.
