WWCC Clearance number (if applicable) …………………………………………………………… 1. You are operating under the supervision of a staff member at all times. Follow staff directions. 2. Abide by all relevant Work, Health and Safety legislation while attending the school or school activity. 3. On arrival proceed immediately to reception and sign in prior to any work starting. You will be issued with and expected to wear a visitors ID tag / badge for the individual school as this is what staff and students are trained to observe. 4. Only use adult toilets. Do not enter student toilets or change areas. 5. Do not use school canteen facilities. 6. Do not smoke or drink alcohol on the school premises or at the school activity. 7. It is not acceptable to attend a school activity or site intoxicated or under the influence of any substance. 8. If a student approaches you or you witness student behaviour that concerns you, immediately advise a teacher in the area or the Principal. 9. It is not acceptable to be alone with any student (unless approved for educational purposes by your supervising teacher) or exchange personal details with a student. 10. Do not exchange any phone numbers with any student, or connect with them through any social media service, (unless approved for educational purposes by your supervising teacher). 11. Maintain adult boundaries with all students and staff. 12. Respect the privacy and confidentiality of students and staff information that you become aware of in your role. IMPORTANT: As a volunteer who is engaging with a school your conduct is subject to a high level of scrutiny to ensure a safe school environment for all. Your continuing role as a volunteer and your attendance on a school site remains subject to the approval and discretion the Principal. Breech of this undertaking may result in action including the removal from the school or school activity.

I understand these requirements and I undertake to adhere to them; I also declare that I have not been charged or convicted of any criminal offence (in Australia or overseas) involving a child (person under the age of 18 years); I have never been accused of any sexual, physical or emotional abuse of a child, nor of neglect or apprehended violence against a child; I have never been the subject of a complaint relating to a breach of professional boundaries; There is no other reason that I might be considered unsafe to work with children; I have received an induction by the school in relation to safe school expectations; I have read the SCS Child Safe Schools Resource Kit – Volunteers; If any of these above declarations change, I will immediately inform the Principal.
