Dear District Volunteer:

Thank you for giving your time by volunteering in our School District. We have been very fortunate to have dedicated, skilled volunteers working in the Hanford Joint Union High School District throughout the years. In an effort to make sure that our volunteers are aware of the School Code requirements and to insure that a safe environment is maintained for our students, we are asking that all volunteers complete the attached volunteer application. Upon completion please return this form with the required documentation to your site contact who will forward it to the site principal’s office. The Principal’s Office will submit the form to Human Resources for final approval. Once approved, you will be contacted to come to the District Office to obtain your Live Scan form for fingerprint clearance. A database of all registered volunteers in the District is maintained in the Human Resources Office and distributed to each site on a monthly basis. I would ask that you take a few minutes to review the attached information and to fully complete this form. All information is confidential and will not be viewed by faculty or staff beyond the Human Resources Office. We appreciate your cooperation with this procedure. Volunteer Information 1. The Governing Board encourages parents/guardians and other members of the community to share their time, knowledge and abilities with our students. 2. Volunteers shall act in accordance with the district policies and regulations, as well as individual site requirements. 3. Each volunteer is required to complete the District Volunteer Application Form, and the completed form as well as all clearances must be on file in the Human Resources Office by commencement of service. 4. Each volunteer must submit evidence he/she is clear from active tuberculosis. TB testing must be up-to-date. The TB testing is at the expense of the volunteer. 5. Each volunteer is expected to maintain confidentiality regarding each student and family. 6. If volunteers hear about or observe evidence of child abuse, they will report the information to the site principal or designee immediately. 7. Education Code 35021 provides that a person who is required to register as a sex offender pursuant to Penal Code 290 shall not serve as a volunteer/chaperone for any field trips or other school activities. 8. Fingerprint clearance is required for all volunteers. Fingerprinting helps the District maintain a safe environment for students and staff. Volunteers are printed using the Live Scan machine at the Kings County Office of Education. The prints are sent to the California Department of Justice Database and the FBI Database. If the fingerprints are in the databases and there has been an arrest or conviction, the District receives this information confidentially. Information about subsequent arrests and convictions is also sent to the District. 9. Certain types of convictions disqualify an individual from serving as a school volunteer/chaperone due to safety concerns. If a volunteer/chaperone is disqualified or if the District receives a subsequent arrest notification from the California Department of Justice or FBI the volunteer/chaperone will be removed immediately and receive notification from the District. 10. If you are also serving as a Volunteer Driver, you will need to submit an H-6 (obtained from the Department of Motor Vehicles) and Proof of Insurance at the time of submission of your volunteer application. 11. If you are a assisting as a volunteer coach in an athletic program, this process is not applicable to coaching positions. 12. Upon receipt of fingerprint clearance, TB skin test, (H-6 and proof of insurance, if driver) the Volunteer will be notified by the District’s Human Resources Office that they are now an approved Volunteer for the District. 13. William L. Fishbough, Superintendent FREQUENTLY ASKED QUESTIONS –FINGERPRINTING Who needs to be fingerprinted? Fingerprint clearance is required for all volunteers in the Hanford Joint Union High School District. Volunteer applications must be signed by the site advisor and site principal. The site principal will then forward the application to the HJUHSD Human Resources Office for the signature of the Director of Administrative Services. Why is fingerprint clearance necessary? Fingerprinting helps the District maintain a safe environment for students and employees. Certain types of convictions disqualify an individual from serving as a school volunteer due to safety concerns. How does fingerprinting work? All fingers are printed using the Live Scan machines available at the Kings County Office of Education. The prints are sent to the California Department of Justice and the FBI. If the fingerprints are in the databases and there has been an arrest or conviction, the District receives this information confidentially. Information about subsequent arrests and convictions is also sent to the District. What do I need to do to get fingerprinted? You will be contacted by the Hanford Joint Union High School District HR department to schedule an appointment to complete the Live Scan Request form that must be taken to the Kings County Office of Education. The Hanford Joint Union High School District HR department, 583-5901, ext. 3110, located at 823 W. Lacey Blvd., Hanford, is open during regular business hours from 7:30 a.m. – 4:30 p.m. except holidays. Once you have received this Live Scan Request form from the district you will then be responsible for making an appointment with the Kings County Office of Education at 584-1441 to have your fingerprints scanned. You will need to pay the $14.00 fee that the Kings County Office of Education charges to process your fingerprints. This $14.00 is due at the time of your fingerprint appointment payable by cash or money order. The District pays the additional fees. You will be notified when clearance has been received. Legal Orientation When you schedule your fingerprint appointment with the Kings County Office of Education, the receptionist will also schedule you for the next available Legal Orientation class. The legal orientation covers sexual harassment, child abuse reporting, and safety in the workplace. This class is a requirement for all employees and volunteers in the Hanford Joint Union High School District. Classes are scheduled through KCOE and are held from 2:30 pm – 4:00 pm on the assigned date at KCOE. TB Skin Tests California Education Code 49406 states that all volunteers must submit evidence of an examination with the past 60 days to determine that he/she is free of active tuberculosis. Volunteers who skin test negative shall thereafter be required to take tuberculosis test every four years in accordance with Education Code.

I declare under penalty of perjury under the laws of the State of California that I have completed the above information truthfully; and have read, understand, and will comply with district requirements and expectations for all volunteers.
