General Overview of Hospital and Facilities

Roger Williams Medical Center (RWMC) is a University affiliated community hospital that overlooks downtown Providence, Rhode Island within easy highway access. Rhode Island is located in the southeastern corner of New England within easy traveling distance of several major northeast cities: 60 miles from Boston, 180 miles from New York City. The Center includes Roger Williams Hospital, Academic Departments, Research Laboratories, Elmhurst Extended Care Facility and the Radiation Therapy suites. RWMC is a 220 bed general medical and surgical hospital which provides primary and tertiary care. The Medical Service occupies 120 beds including: o

The faculty at RWMC specialize in various subspecialties of medicine, including neurology, gastroenterology, rheumatology, dermatology, oncology, substance abuse, endocrinology, and infectious disease. There are over 6,700 admissions to RWMC per year, the majority through ER. All medical patients are admitted to the house staff teaching service and all orders are written by house staff. As a part of the internal medicine team, medical students are provided with the practical experience of admitting patients, learning to take histories from patients and perform physical examinations under the supervision of housestaff. Attending’s and consultants contribute to management discussion but all care is directed by and ordered by the house officers. Many procedures are done by certified house staff and medical students are able to learn hands-on skills, with the appropriate supervision.

FACILITIES: Library The Medical Library at the hospital is open 24 hours a day and staffed during regular working hours. It stocks more than 150 medical and scientific journals and maintains a large collection of scientific, medical and nursing textbooks. Free unlimited photocopying is available 24 hours a day for housestaff and medical students. (Code: 244)

Computer access to the National Library of Medicine collection via the Internet is available 24 hours a day. The hospital Librarian can obtain print copies of references not available within the hospital without charge, usually within two days. All hospital computers provide access to educational resources such as Up to Date and the New England Journal of Medicine. On-call Rooms All house staff and medical students participating in long-call are provided an individual call-room with a single bed, desk, side lamp and access to full showers and facilities. The call-room is available during the daytime hours to all medical students who require a secure place to store their personal belongings. There are also lockers available on the second floor of the Simpson building next to Medical Education for the use of medical students and housestaff. Housestaff Lounge the medical students are invited to share the housestaff resources, including a brand new lounge with television, computers, water-cooler, microwave, refrigerator and call room. Parking There is free-parking available to medical students, located at the front of the main entrance to the hospital. There are no designated parking areas for housestaff and medical students; however there is usually ample parking in the morning hours. Cafeteria RWMC has a fully staffed cafeteria with fresh meals provided daily to all staff and visitors. There are always healthy and vegetarian options available to customers. There is not a meal allowance for medical students, however there are often free lunches provided at the noon conference several times each week. After hours there are vending machines available for snacks and beverages only. II. BASIC INFORMATION

Office: 2nd floor Prior room 203, pager: Dr. Bernard Zimmermann, Program Director, Department of Medicine: 401-456-2070 Orientation: Held at 9-9:30 am on first day, at the Dr. Macko Medical Library. At this time team assignments will be made, discussion of monthly schedule, clinical responsibilities and learning objectives for the rotation. Students will be given an orientation of the hospital and shown relevant facilities.

III. INTERNAL MEDICINE LEARNING OBJECTIVES For a complete description of all learning objectives, students should refer to the UNECOM Internal Medicine clerkship syllabus. For third year medical students, a hospital based experience in general internal medicine is essential to prepare for additional clerkship experiences. The core clerkship in medicine is designed to give students a broad exposure to all areas of internal medicine. Through participating as a part of the medical team, students should be able to develop skills in the following areas: 1. Interviewing skills -the ability to obtain and record an accurate history with relevant questioning. Emphasis on obtaining a complete history including presenting illness, medications, allergies, social and family histories, complete review of systems. 2. Physical examination -emphasis on developing the necessary skills in physical examination to guide patient assessment. 3. Differential diagnosis -the ability to formulate a differential diagnosis based on history and physical examination findings and patterns of disease presentation. 4. Recommendation of initial diagnostic studies -based on clinical findings and differential diagnosis. Emphasis on interpretation of laboratory and diagnostic studies will also be made. 5. SOAP note/progress notes -the ability to complete daily progress notes and accurately document history and examination findings is essential. Residents and interns will be responsible for reviewing student notes on a daily basis to assist in development of documentation skills. 6. Interpersonal skills-the ability to establish a healthy physician/patient relationship with emphasis on attitude, demeanor and appearance. Students are encouraged to recognize their importance as part of the medical team and further develop communication skills through working alongside nursing and ancillary staff. 7. The ability to identify and apply preventive screening and health promotion activities for the patient.

SKILLS DEVELOPMENT Certain skills and aptitudes should be developed through ward experience and didactic lectures. Some of these include: o Interpretation of EKG rhythm, blocks, ischemia, injury and infarct patterns o Arterial blood gases/acid base balance o Venipuncture and peripheral IV access o Urinary catheterization o Screening Pulmonary Function Tests o Interpretation of routine hematological studies o Interpretation of routine blood chemistries o Interpretation of urinalysis o ACLS protocols o Interpretation of blood, wound, sputum, stool, urine cultures as well as MIC. Familiarity and observation of procedures including: o o o o o o

Endoscopy Non-invasive cardiovascular studies (MUGA, Nuclear stress tests, Echocardiography) Biopsy procedures, paracentesis, thoracentesis Lumbar puncture Imaging studies Joint aspiration and injection

IV. STRUCTURE OF WARD TEAM AND ROLE OF MEDICAL STUDENTS Medical students are assigned to become an integral part of a general medicine ward team. Our residency program handles all admissions to the medicine service. There are 7 'GMF' teams that participate in both long call and short call. There is no overnight call in the hospital. GMF team: Long call for each ward team, made up of a resident and an intern, is every seventh night. The team admits on these days until 9:00 pm. The intern on each team may admit up to 5 new patients. The resident on the GMF team may admit an additional 5 patients per call. The long-call team signs out to the intern night float once they are done their admissions for the evening. They return to the hospital in the morning for a regular day, but have no new admissions. When the team is on 'short call', they are available for admissions until 6pm. On other days of the week the team signs out to the intern float at 4pm. ROLE OF THE MEDICAL STUDENT The medical student serves as an integral role of the medical team. As a third year medical student, you will be performing history and physical examinations of new patients, interpreting laboratory studies and developing a differential diagnosis and plan alongside your intern/resident team. The goal as a medical student is to become adept at obtaining H/Ps and to develop your skills at providing differential diagnoses and management plans. It is also important to develop documentation skills (writing SOAP notes) and communication skills through presenting patients on daily work rounds. You will be responsible for seeing 'your' assigned patient in the morning, collecting vital signs and laboratory data prior to team rounds. The intern/resident on your assigned team is expected to review and discuss all of your findings on a daily basis. There should be time set aside daily to discuss your note, the management plan and for teaching of your case. You are not expected as a third year student to know what exactly the management strategy will be. Being attentive to your patient, conscientious and prepared is as important as book knowledge! We hope that the medical students will also gain experience with performing minor procedures (such as ABGs) with appropriate supervision. As a fourth year (subintern), you will be given additional patients to follow and will act in the same role as an intern, with guidance and support. This particular rotation helps to prepare for a medical internship.

LECTURE AND CONFERENCE SCHEDULE 1. Morning Report -daily (Mon-Fri) from 10am-11am In Dr. Macko Medical Library. Medical students should attend with their resident. This is a forum for residents to present their patients and for further discussion facilitated by the Medical Faculty. 2. Attending Rounds -occurs three times weekly (Mon/Wed/Fri) from 11:00am-12:00pm. Intern/Resident/Medical student attends. The group presents current patients and discusses with their assigned Attending. Interns and medical students are encouraged to present patients as they do not have this opportunity at Morning Report. The format is flexible and may include bedside rounding on patients, discussion of papers, or didactic teaching. 3. Noon Conference -occurs daily (Mon-Fri) from 12:00 – 1 pm in Kay Auditorium C (basement). Variety of topics related to general Internal Medicine and subspecialties are covered throughout the year. 4. Grand Rounds Tuesdays at 11 AM till 12 Noon in the Kay Auditorium (beginning September). An updated list of monthly conferences is listed on the bulletin board outside Paolino conference room. Housestaff attendance is mandatory.
