This form is to be executed by the Association Board of Directors, if over 50% of the membership executed the Consent not to retrofit. This form must be filed in the Public Record with the Clerk of Court. 3) Notice of the Results of the Opt-Out Vote. To be executed by the Association President, attesting that the unit owners have indeed consented to forego Retrofitting. 4) Proof of Notice Affidavit. Executed by the Association stating that the results of the Opt-Out Consent forms were mailed to all unit owners. 5) Retrofitting report for Condominiums. (Form CO 6000-8) Upon completion of the first four steps, this form must be filed with the Division of Florida Condominiums, Timeshares, and Mobile Homes.

NOTICE OF THE RESULTS OF THE OPT-OUT VOTE TO FOREGO RETROFITTING Attention Unit Owners, The undersigned President of this corporation, in accordance with Section 718.112(2)(1), Florida Statutes, attests by this notice that the unit owners of this Association have voted to forgo retrofitting the common elements, association property, or units with a fire sprinkler system by the written consent of a majority of all voting interests in the condominium. This vote is evidenced by the "Certificate of Vote to Opt Out of Fire Sprinkler Retrofit" which was duly recorded in the Public Records of Palm Beach County, Florida, in Official Records Book ,atPage Note: A copy of this notice must be provided by the current owner to a new owner before closing and by a unit owner to a renter before signing a lease.

Notwithstanding the membership approval to waive the retrofitting requirements, as per Section 718.112(2)(1)(2) Florida Statutes, a vote to rescind this waiver and to require retrofitting may be obtained at a special meeting of the unit owners called by a petition of at least 10 percent of the voting interests. Such a vote may only be called once every 3 years. Notice shall be provided as required for any regularly called meeting of the unit owners, and must state the purpose of the meeting.
