TRIATHLON AUSTRALIA TRAVEL INSURANCE 2012/2013 INTRODUCTION Willis insurance brokers have worked with Triathlon Australia and their affiliated State Associations to provide travel insurance for nominated members, officials and employees. This brochure is a summary of the cover that has been organised on your behalf. If you require additional cover, please contact Willis for an individual quotation. WHO IS WILLIS?

EXCLUSIONS

Willis is a licensed insurance broker and has organised this insurance program in association with Triathlon Australia. Please refer to the Policy Wording for a full list of exclusions via the Travel Policy. The main exclusions effecting Triathlon Australia members are as follows; Pre-existing Condition means: 1) in respect of Injury: a condition with which the Insured Person was aware of (whether diagnosed or not) or has sought treatment prior to the Insured Travel covered under this Policy.

WHO IS INSURED? Triathlon Australia annual members (including professional license holders), officials, coaches, employees and volunteers whilst engaged in authorised travel.

2) in respect of Sickness:

i. a condition or side effect with which the Insured Person was aware of (whether diagnosed or not) or has sought treatment prior to the Insured Travel covered under this Policy. If any form of cancer is a Pre-Existing Condition, then there is no cover for cancer or cancer-related conditions.



ii. a condition caused by a Pre-Existing Condition, that is, any medical condition that you have suffered from or been treated for, irrespective of whether a complete recovery has occurred.

INSURER The insurer of this group travel insurance policy is CGU Insurance Limited via Accident & Health International Underwriting Pty Ltd (AHI), GPO Box 4213, SYDNEY NSW 2001. HOW TO MAKE A CLAIM •

 otify the team manager (if appropriate) of the loss immediately and N request that the loss is reported to the organisers (if appropriate). Members not participating in a team should contact Dynamiq Assist on +61 2 9978 6666 directly, reverse charge calls will be accepted.

Accident & Health International claims are handled by Dynamiq assist. Please contact Dynamiq Assist as soon as possible if you need or are receiving medical treatment. You or your physician should contact Dynamiq Assist prior to any evacuation or repatriation, reverse charge anywhere in the world.
