chronic, recurrent and relapsing depression in South Yorkshire Specify factors and techniques that predict best outcomes and service quality for people with longer term depression (LTD) Demonstrate collaborative provision consistent with empowerment, ‘expert patient’ and recovery models Produce clinical, training and management support tools for system improvement

Three linked projects • System modelling: – start with a conceptual model of psychological services care pathway and move via research review towards quantified decision model

• Understanding self-management:

– learn from people managing long term depression what they find helpful & need from services

• System improvement:

– test ideas generated from research & service users, test impact of service improvements

Using the Modelling • The modelling will allow us to explore which • • •

elements of this care pathway are responsive to patient self-management It will help us to identify the comparative costs and benefits of improving different parts of the pathway This and the next two workshops will generate ideas and agree priorities for improving the pathway Those with the greatest potential will be tested in the cohort study

Developing the Cohort • People experiencing LTD in a community will be

recruited to the cohort study over two years They will be recruited at the point of service use in the Improving Access to Psychological Therapies service, the relevant Community Mental Health Team and tertiary care services Service use, symptoms, functioning and satisfaction will be monitored for the cohort for up to five years The data from the cohort will assist with enhancing our understanding of service use and longer-term outcomes for people with LTD The data will form the backdrop to evaluating agreed innovations

Testing Innovations • The innovations identified this afternoon will •

form the starting point for developing clinical, training and management support tools The modelling work will identify those with greatest potential to enhance the care pathway The next two workshops will further develop and prioritise those innovations to test Following consultation and agreement the selected innovations will be implemented within the cohort sites sequentially Quantitative data and qualitative feedback will provide an initial evaluation of the innovations
