PERMISSION TO PARTICIPATE AND RELEASE OF LIABILITY I, _________________________, am the parent or legal guardian of ____________________, a minor. I give permission for my daughter/son/ward to participate in a reality-based educational experience that may consist of assembling, manufacturing, or otherwise creating products for sale. I understand that such participation may include the use of tools, machinery, or other devices. Activities my daughter/son/ward engages in is at her or his discretion while participating in the educational experience (the “Classroom Marketplace”). I. RELEASE AND WAIVER OF LIABILITY A. In consideration of my daughter/son/ward’s being given an opportunity to participate in the Classroom Marketplace, I hereby release and forever discharge and hold harmless Real World Scholars and their respective officers, directors, employees, agents, successors and/or assigns (collectively “RWS”) from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from participation in the Classroom Marketplace. B. I understand that RWS is not responsible for supervision of my daughter/son/ward and is in no way responsible for the teachers, educational officers, or other agents of the school district or individual school that oversee the Classroom Marketplace. Nevertheless, I understand that my child will be expected to follow any supervisors, teachers, or other such agents’ instructions and reasonable rules of behavior and failure to do so may result in an immediate dismissal from the Classroom Marketplace with no liability extending to RWS. I HAVE CAREFULLY READ THE FOREGOING AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I HAVE HAD AN OPPORTUNITY TO ASK QUESTIONS AND NEGOTIATE REGARDING ITS CONTENTS. I AM AWARE THAT THIS AGREEMENT INCLUDES A WAIVER OF LIABILITY AND AN ASSUMPTION OF RISK AND I SIGN IT OF MY OWN FREE WILL.
