QUIXOTE VILLAGE RENTAL AGREEMENT

This Rental Agreement is made and executed in duplicate this day of , by and between Panza, (3350 Mottman Road SW, Olympia, Washington) a nonprofit corporation, the owner and manager (hereinafter called the “Landlord”) and ___________________________ (hereinafter called the “Resident”). In consideration of the payment of the rents and the performance of the covenants contained herein on the part of the Resident and the Landlord, the Landlord does hereby rent to the Resident and, in consideration of the premises, the Resident does hereby rent from the Landlord upon these terms and conditions, the following property particularly described as follows: 1) 1) Property: Cottage Number _____ located at 3350 Mottman Rd.SW, Olympia, Washington. 2) Term: The term shall be month to month commencing on this date:______________. 3) Rent: Monthly rent shall be 30% of the Resident’s income per month due on the first of each month unless an alternate monthly due date is approved by the Landlord. Rent will be considered late if received by Landlord more than five days after the 1st of the month or the alternate monthly due date. Initial rent is $____________ Security Deposit: Resident will: Pay a security deposit of $450.00 to the Landlord, which will be deposited in an account at Sterling Bank (3000 Harrison Ave NW, Olympia, WA 98502) as security for the faithful performance of the terms of this agreement. The security deposit is due and payable in full prior to move-in to the Cottage, unless Landlord shall agree to installments, which shall be as follows: ___________________________________________. Landlord shall hold this security deposit for the period Resident occupies the unit. After Resident has moved from this unit, Landlord shall determine if Resident is eligible for a refund of any or all of the deposit. The amount of the refund will be determined in accordance with the following conditions and procedures. a. Resident will be eligible for a refund of the security deposit only if Resident provided Landlord with the twenty (20) day written notice of intent to move as provided herein. b. After Resident has moved from the cottage Landlord will inspect the cottage and complete cottage inspection report. Landlord will permit Resident to participate in that inspection if Resident so requests.

c. Landlord will refund to Resident the amount of the security deposit, less any amount needed to pay the cost of any of the following: i. Loss due to Resident’s failure to pay all of the rent owing; ii. Cost of repairing damage to premises in excess of normal wear and tear, including the removal of trash or personal items; iii. Loss of rent due to time needed to repair excessive damage; iv. Costs to replace or repair furniture, equipment, or other items belonging to Landlord and taken and/or damaged by Resident and/or their guest(s); and/or v. Charges for any unreturned keys, late payments or returned checks as described herein. d. Landlord agrees to refund to Resident, within fourteen (14) days after Resident permanently vacates the cottage, any amounts due under the terms of this paragraph. e. The Security Deposit is not to be deducted by Resident from rent payments or other charges due hereunder, and Landlord’s right to possession of the premises for non-payment of rent or for any other reason shall not be affected by the fact that Landlord holds these deposits. 4) ADA Rider: Any resident occupying an ADA-compliant cottage must be prepared to move to an available non-ADA compliant cottage as soon as possible if someone requiring a ADA-compliant cottage is a successful applicant for residency. 5) Utilities: The following utilities are included in the monthly rent and paid by building management: electricity (including heat), water, sewer, and garbage. Telephone and Cable TV are the sole responsibility of resident. 6) General Restrictions: Resident must occupy the premises. Resident shall use the premises as a private dwelling for himself or herself only. This Rental Agreement is being agreed to for the occupancy of the following person only: Name: _____________________________________ Resident Resident may not receive mail for any persons other than those listed in this lease agreement. Resident cannot sublet or assign the cottage or any part thereof, or any interest therein without the written authorization of Landlord. Landlord may withhold this consent at Landlord’s sole discretion. 2

Resident and all guests shall abide by the Quixote Village Rules and Policies as set forth in Attachment 2. Failure to do so shall be grounds for eviction. Resident and guests shall have due regard for the peace, comfort and enjoyment of the other residents in the Village. 7) Additional Charges: In addition to the basic rent, and as additional rent, the following charges will be made: a. Resident shall be in default under this agreement if the rent is not paid in full by the end of the fifth (5th) day of the month or the end of the 5th day of the agreed monthly due date, and agrees to pay a fee of $20.00 for each breach of this condition to compensate Landlord for the administrative expenses incurred in processing the late payment. If rent is paid 10 or more days late and is accepted by Landlord, an additional $40.00 late fee will be charged. b. Resident agrees to pay a charge of $25.00 for each returned unpaid check to compensate Landlord for the administrative expenses incurred in processing the returned check. Only a cashier’s check, cash or money order will be accepted to honor the returned check. c. If a check is submitted without sufficient funds two (2) times, all payments for the next twelve (12) months must be made with a cashier’s check, cash or money order. 8) Condition of Dwelling Unit: By signing this Rental Agreement, Resident acknowledges that Resident has inspected the cottage and that the cottage is safe, clean, and in good condition. Resident accepts the cottage in its present condition and agrees that all appliances and equipment in the unit are in good working order, except as described on the Unit Inspection Report, which is Attachment 1 to this Rental Agreement. Resident also agrees that Landlord has made no promises to decorate, alter, or improve the unit, except as listed on the cottage Inspection Report. Resident also acknowledges that Quixote Village is located in an area zoned for industrial use and that they may experience a noise levels appropriate to activity in such a zone. 9) Resident Obligations: Resident agrees to abide by the Quixote Village Rules and Policies, which are included in this rental agreement as Attachment 2: 10) Restrictions on Alterations: Resident agrees not to make any alterations to the cottage including, but not limited to, any of the following without first obtaining Landlord’s written permission: a. Change or remove any part of the appliances, fixtures or equipment in the unit;

b. Paint or install wallpaper or contact paper in the unit; c. Attach awnings or window guards to the unit; d. Build or attach any structure to the walls or ceiling in the unit; e. Remove windows or window screens for any reason. Landlord has no obligation to consent to alterations, except as required by applicable local, State, and Federal Fair Housing Laws. 11) Keys and Locks: Landlord agrees to provide to the Resident all applicable keys. Resident shall not install additional or different locks or gates on any doors or windows. Resident shall not give keys referred to herein to non-residents. When this Rental Agreement terminates Resident shall return all keys to the dwelling unit and mailbox to Landlord. Landlord will charge Resident $15.00 for each key not returned. Keys shall not be duplicated for any reason except by Landlord. In the event locks are changed at Resident’s request, Resident shall be charged actual cost for such replacement. This charge shall not be added to Resident’s rent payment. a. Access by Landlord: Landlord agrees to enter the cottage only during reasonable hours, to provide reasonable advance notice of a minimum of 48 hours of Landlord’s intent to enter the cottage, and to enter the unit only after receiving Resident’s consent to do so, which consent shall not be unreasonably withheld, except when emergency situations make such notices impossible or except under Paragraph (c) below. b. Resident shall permit Landlord, its agents, or other persons, when authorized by Landlord, to enter the cottage for the purpose of making reasonable repairs, improvements, and periodic inspections. c. After Resident has given a notice of intent to move, Resident agrees to permit Landlord to show the cottage, on reasonable notice, to prospective tenants during reasonable hours. d. If Resident vacates the unit before the end of any monthly rental period, Landlord may enter the cottage to decorate, remodel, alter, or otherwise prepare the unit for re-occupancy. 12) Holdover: In the event either the Landlord or Resident terminates this agreement using a twenty day notice as provided herein, Resident agrees to vacate the premises before 10:00 p.m. on the last day of the monthly rental term. Should Resident fail to vacate on or before the expiration or termination date, the holdover shall be presumed to be willful and deliberate, and Landlord shall be entitled to require Resident to vacate the unit and take possession of the premises, as well as to damages incurred

through the loss of a prospective Resident, other expenses incurred due to breach of this condition of the Rental Agreement, including but not limited to, attorney’s fees. 13) Attorney’s fees: In the event that it is necessary for either party to retain an attorney or to bring suit to enforce this agreement, the prevailing party shall be entitled to reasonable attorney’s fees service fees, and court costs reasonably required to enforce the agreement. 14) Abandonment/Default: Resident’s absence from the premises for 15 consecutive days while all or any portion of the rent is unpaid shall be deemed an abandonment of said premises unless Resident has indicated otherwise to Landlord, and this tenancy shall, at the option of the Landlord, terminate without further notice. In addition, any default under this Rental Agreement shall entitle Landlord to terminate this Rental Agreement and evict Resident. In such event, Landlord may dispose of all Resident’s property remaining on said premises in a manner consistent with the prevailing Landlord/Tenant law and re-rent said premises without liability to tenant whatsoever. Furthermore, in the event rent is fully paid, the absence of a Resident from the premises for more than 30 consecutive days or absence from the premises for more than 60 days during a calendar year shall be grounds for termination of this Rental Agreement unless Landlord agrees in writing to an exception to this requirement. 15) Resident’s and Landlord’s Liabilities: Resident understands that it is Resident’s choice whether to obtain renter’s insurance covering personal property, and Resident further agrees that all personal property in the cottage and any designated storage areas shall be at the risk of the Resident. Resident releases Landlord from any liability for loss of damage to Resident’s property while located in the cottage or in any other part of the Village unless such damage is caused by Landlord’s gross negligence. No property shall be stored outside the cottage. Resident shall remove all property from the premises immediately upon termination of tenancy. In the event such property is not so removed, Landlord may dispose of same without any liability to Resident whatsoever. Resident releases Landlord from any and all liability to person and property suffered by Resident while on said premises or on the premises of which the cottage is a part. Resident shall hold Landlord safe and harmless from liability for any injury or damage to any person or property resulting from the acts, omission, or negligence of Resident or Resident’s guests. 16) Resident’s Notice to Vacate to Landlord and Landlord’s Notice to Resident to Vacate Either party may terminate this Rental Agreement by giving twenty (20) days written notice, which shall be given at least 20 days or more before rent is due. Landlord may terminate this Rental Agreement as otherwise provided by law. 17) Vehicles: Parking is available on a first come first served basis. No parking spaces are reserved except as designated for disabled. Residents are limited to one vehicle per cottage. The vehicles must be registered with the management. All vehicles must maintain current registration and be currently in use to remain parked in the parking lot and street. All vehicles must be operable and street legal, i.e. no flat tires,

broken windows, supports, etc. No vehicle maintenance of any kind is permitted in the parking lot and street. Any violation of this paragraph may result in towing of the vehicle at owner’s expense. 18) Non Waiver: Landlord’s action or inaction shall not be deemed to be a waiver of any provision of this agreement unless done in writing. 19) Contents of the Agreement: This Rental Agreement and the following Attachments make up the entire agreement between the Resident and the Landlord regarding the unit. If any court declares a particular provision of this agreement to be invalid or illegal, all other terms of this agreement shall remain in effect and both Landlord and Resident will continue to be bound by them. RENTAL AGREEMENT ATTACHMENT 2 Quixote Village Rules and Policies

Quixote Village Rules and Policies Program Overview Quixote Village is the brainchild of a group of homeless people who founded Camp Quixote. They wanted the privacy and dignity of their own house, but they wanted to live in community with each other. That’s why they envisioned 30 small cottages and a central community building with showers, laundry, and kitchen facilities, and living and meeting space.

A few words about rules • •

• •

Our goal is to have as few rules as possible, and to enforce all the rules fairly. Some rules and policies are made by Panza in its role as landlord; others are made by the Resident Council. Still other rules and policies have been made jointly by Panza and the Council. Rules made by Panza can only be changed by Panza; rules made by the Resident Council can only be changed by the Council. Rules made jointly can only be changed by joint action. Panza relies on the Resident Council to fairly implement some of the rules made by Panza. If the Resident Council fails to do so, Panza will intervene to ensure that all rules are fairly enforced. In the unlikely event that the Resident Manager and/or Panza and the Resident Council cannot reach agreement on an issue deemed to be one concerning the health and safety of Quixote Village and its residents, the Board of Directors of Panza will have ultimate authority in the matter.

As a condition of admittance to residency in Quixote Village, each applicant will be required to sign a Residency Rules Agreement that is consistent with the rules and policies in the remainder of this section of the management plan. Resident Council Rules and Policies The following rules and policies have been developed by the Resident Council for the self-governance of Quixote Village.

Cardinal Rules These five most basic rules were originally established by the residents of Camp Quixote. They have stood the test of time. Violation of any of these cardinal rules is grounds for eviction from Quixote Village. No violence or the threat of violence, abusive language, gestures, or behavior; No expressions of prejudice or disrespect toward or about anyone based on race, ethnic background, religion, age, disability or sexual orientation; No sexual or any type of harassment by residents, visitors or staff; No weapons or firearms; No illegal behavior of any kind. Resident Work Contribution Quixote Village is a low-budget operation, and everyone’s help is needed to keep costs down so that the Village can be sustained. •

All residents contribute a minimum of eight hours of service to the Village each week by doing assigned chores intended to maintain common areas, such as the kitchen, shower and laundry areas, and landscape. These tasks will be organized and assigned by the Resident Council’s Executive Committee. The number of required hours may be changed from time to time by the Resident Council, depending on how much work needs to be done.

Visitor Policies •

All visitors must be signed in at the front desk by the resident they are visiting. The resident is responsible for the visitor, must be with them during the entire visit, and must sign the visitor out when he or she leaves. The resident is responsible for any violation of Village rules by their visitor.



Overnight visits are limited to three nights per month; exceptions to this rule may be made by the Resident Council.



No visitor may stay overnight until he or she has passed a background check.



Visitors are welcome to arrive between 9 AM and 10 PM. (Visitors may stay past 10 PM, but they need to arrive before then.)

Visitors may not bring any illegal drugs to the Village, and doing so will be grounds for being permanently banned.

Quiet Hours •

All residents must be quiet from 10 PM until 8 AM. This means that no noise from one cottage should be audible in another. Noise in the community building should not be audible outside it. Noise in the outdoor area should not be audible indoors.

Leaves of Absence •

Residents must tell the Program Manager if they plan to be away from the Village for more than three days. (This rule is simply to ensure that everyone is safe; residents are not required to say where they are going or why.)

Common Area Rules In the Community Building • • • • • •

Residents are expected to protect others’ health by covering coughs, properly disposing of tissues, and thoroughly washing hands when entering the kitchen. Every resident is responsible for cleaning up after themselves in the kitchen, dining area, living room, laundry room and shower rooms. Kitchen cleanup includes rinsing dishes and placing them in the dishwasher, wiping down and sanitizing counters and stovetops, washing, drying and putting away pots and pans, and properly disposing of recycling, compost and garbage. Dining area cleanup includes clearing tables, wiping and sanitizing tables, and leaving tables empty of any clutter. Laundry room cleanup includes removing all clothing, laundry products and containers promptly, and cleaning the lint trap in the dryer after each use. Shower room cleanup includes rinsing and sanitizing tubs and showers, removing all personal care products, towels, and clothing, and cleaning hair out of drains.

On the Grounds • • • • •

All cigarette butts go in containers labeled for that purpose. No littering. Respect residents’ garden areas by not walking in them. Keep front porches reasonably tidy and presentable. Each resident is responsible for the maintenance of the garden in front of his or her cottage.

Garbage, Compost and Recycling

Residents are required to separate garbage, recycling and compost and to dispose of each promptly and correctly.

Consequences for Rule Violations The Executive Committee will work with the Program Manager to document and address all rule violations. Decisions on the consequences of rule violations relating to behavior that threatens the residents' health and safety or timely payment of rent will be made by the Program Manager. All decisions on consequences will be made in accordance with the principle of equal treatment for all. The repetition of offences justifies more serious consequences. The consequences of rule violations may include eviction. Joint Resident Council and Panza Rules and Policies The following rules and policies regarding life at Quixote Village have been jointly developed by the Resident Council and the Board of Directors of Panza. Village Drug/Alcohol Policy Addiction is a disease, and those who are chemically dependent need to be treated respectfully and provided treatment that will help them to get out from under their addiction. Those individuals need a safe and supportive community that encourages them in their recovery. We know that addiction is a relapsing disease and those with addiction deserve opportunities to try to be in recovery, fail, and try again. While we understand the complexity of this disease, we believe that the expectation should be that residents be clean of illicit drugs and alcohol at the time that they move into the Village. These are the requirements for the drug/alcohol policy at Quixote Village: •

Quixote Village will be a welcoming and safe environment for those who are recovering from drug or alcohol addiction.



The common areas of the Village are meant to be a safe place for all residents. Therefore, when in any common area, residents must not consume alcohol, exhibit behavior related to their alcohol use, or even smell of alcohol. If this happens, the resident will be asked to return to their cottage. Residents who violate this rule may be asked to take a breathalyzer test and will face sanctions.



No illicit drug use, possession, distribution, or manufacturing (including possession of illicit drug paraphernalia) will be allowed at Quixote Village. Residents may not use illicit drugs while living at Quixote Village. That means that residents cannot use at any time, whether on or off the property.

UAs and breathalyzer tests may be used at the direction of the Program Manager or the Executive Committee. These tests are intended to be tools for recovery and accountability. They may be used to address problematic behavior of an individual resident, or randomly to ensure that everyone in the community is living by the rules.



Drug dealing and sharing drugs must never happen. If drugs are dealt or shared this will be seen as an extremely serious offense, with strong and immediate action taken. Drug dealing or sharing are selfish acts that directly affect other residents and steal their recovery, their sense of hope for a new life, and their safe haven.



If a resident exhibits behavior that indicates alcohol inebriation, illicit drug use, possession, distribution, or manufacturing, Quixote Village will require that the resident meet with the Resident Advocate to discuss their drug or alcohol-related behavior and the cause of that behavior in the following ways: o If the resident acknowledges that they have been drinking excessively or using, or have been found using illicit drugs or being inebriated in common spaces of the Village, they will be required to develop a collaborative plan to address their drug/alcohol use and behavior. This plan may include UAs or breathalyzer tests as tools for recovery and accountability. This is an opportunity for the resident to create a plan with the help of the Resident Advocate that they believe will work for them. They will have to turn in documentation showing that they are following through with this plan. If they do not follow their plan or the timeline of the plan, or if their drug/ alcohol use or behavior continues, they will then be required to complete a chemical dependency assessment at Providence St. Peter’s Chemical Dependency Treatment Program and to follow the recommendations of that assessment. o If the resident denies illicit drug use or a drinking problem, the Executive Committee, the Program Manager or the Resident Advocate must decide whether to ask the resident to submit to a UA or a breathalyzer. If the test shows the presence of illicit drugs or alcohol, the resident will be required to develop a collaborative plan to address their drug/alcohol use and behavior. This is an opportunity for the resident to create a plan with the help of the Resident Advocate that they believe will work for them. They will have to turn in documentation showing that they are following through with this plan. If they do not follow their plan or if their drug or alcohol-related use and behavior continues, they will be required to complete a chemical dependency assessment at Providence St. Peter’s Chemical Dependency Treatment Program and must follow the recommendations of that assessment. (Just going to meetings like AA meetings will not be a substitute for an assessment.)

Quixote Village will hold a place in the Village for any resident who is referred to in-patient chemical dependency treatment, while in treatment. Rental payment and chore completion will be worked out with the Program Manager.



The Resident Advocate will assist residents in sticking with their aftercare plan when they have completed in- or out-patient treatment.



If the resident refuses to discontinue their alcohol abuse, drug use, drug behavior, drug dealing or drug sharing or decides not to comply with the rules stated above, the Program Manager and Resident Council, with input from the Resident Advocate, may eventually have to ask the resident to leave, especially in cases where the drug use and behavior is affecting other residents. Asking a resident to leave is never the desired outcome and should only occur when someone has been given chances and has shown over a period of time that recovery is not their goal.

Village Governance Panza, the non-profit support organization that serves as the Village landlord, makes rules necessary for the legal, safe, and sustainable operation of the Village. The Resident Council, in which every resident has an equal voice, makes rules regarding resident behavior; its Executive Team imposes disciplinary measures for some rule violations. The Executive Team and the Resident Council may make recommendations to the Program Manager regarding candidates for residency or eviction. Panza reserves the right, as legal landlord, to make final decisions on entry and exiting based on the information at hand, which may include the Council’s recommendations. •

All residents are required to attend weekly Resident Council meetings, and to show up on time and stay until the meeting is adjourned. Valid excuses for not attending are work, medical emergencies, family visits and family emergencies. Written verification may be requested. The resident must notify staff or a Council leader if the resident is unable to attend.



The Resident Council will hold elections on the last Sunday in April and September for a five-member Executive Committee. After two consecutive, sixmonth terms on this committee, a resident must "sit out" one term before being eligible to serve again.



The Executive Committee will be responsible for preparing weekly meeting agendas, facilitating Resident Council meetings, making resident work assignments, reviewing and proposing any rule changes to the Resident Council, and imposing disciplinary measures for some violations of Council Rules. Ongoing training in the basic tenets of the Landlord-Tenant Act will be provided to the members of the Executive Committee by the Program Manager.

The Program Manager will provide staff support and assistance to the Executive Committee and will keep an accurate record of decisions of the Executive Committee and the Resident Council.



The Executive Committee will interview applicants for residence in the Village, and explain to them the responsibilities and rights of residents. Ongoing training in the basic tenets of Fair Housing Law will be provided to the members of the Executive Committee by the Program Manager.



Decisions at a Resident Council meeting are made by a majority of those residents present.



The Executive Committee is authorized to make decisions about Village discipline and order, except those reserved for the Program Manager and/or Panza, between meetings. Residents may appeal any decision of the Executive Committee at the next Resident Council meeting. However, the Executive Committee’s decision stands until the next meeting.



Elected leaders of the Village will attend monthly Panza meetings to represent Village residents. They will report on the Panza meeting at the next Resident Council meeting.



All residents are required to assist at occasional fundraising events, and are encouraged to attend public meetings where the Village should be represented.



Those who are comfortable doing so are encouraged to speak on behalf of the Village residents at community events. However, no one is allowed to represent the residents without advance permission of the Village’s elected leaders or its Resident Council.

Panza Rules and Policies The following rules and policies have been developed by the Board of Directors of Panza in their role as landlord of Quixote Village with final responsibility for the health and safety of its residents. Evictions The Program Manager will have sole authority to evict residents. The Executive Committee may play an advisory role on these decisions. Residents who are evicted and wish to appeal may do so at the next regularly scheduled meeting of the Panza Board, and the decision of Panza will be final. General Rules

No cooking or perishable food storage is allowed in the residents’ cottages.



No kitchen appliances (toasters, crock pots, microwaves, refrigerators, etc.) except coffee makers are allowed in the cottages.



No over-use or unsafe use of electrical outlets is allowed, and all residents are expected to turn off heat and lights when they leave their cottages.



No tobacco smoking is allowed inside cottages or the community building.



No nails or brackets of any kind are allowed in cottage walls; the Program Manager will provide approved methods for such things as hanging pictures and installing shelves.



Residents are expected to keep cottages reasonably clean. This means emptying all garbage and recycling at least once a week, cleaning toilets and sinks, and limiting possessions to what can be safely and reasonably stored inside the cottage. The Program Manager and a representative of the Executive Committee will inspect each cottage once a month to ensure compliance with this requirement. Residents will be notified 48 hours in advance of a monthly inspection.



All residents’ belongings must be contained inside their cottages, and front porches must be kept tidy, and may not be used for storage.



Residents may not build any accessory structures or make any alterations to cottages.



Residents must immediately inform the Program Manager of any defects or damage to plumbing, fixtures, heater, or any other part of the cottage.



Residents must pay the actual cost for disposal of any large items, such as furniture, building materials, or mattresses.

Resident Goals •

All residents will meet with the Resident Advocate prior to moving in and establish goals they will work to achieve while in the Village.



Residents who come to the Village with an active addiction must include becoming clean and sober as one of their goals, and agree to be held accountable for achieving this goal. Periodic drug and alcohol testing may be conducted to ensure honesty and accountability.

Once in the Village, residents agree to meet with the Resident Advocate at least once a month to review progress towards meeting their goals.



Lack of serious and consistent effort by a resident towards meeting personal goals will result in penalties, including eviction.

Bedbug/pest Policy To prevent the spread of bedbugs and other pests, all residents must be vigilant and report any sign of infestation to the Program Manager immediately. Residents must also abide by the following rules to prevent infestations: •

Do not EVER remove the mattress covers. If they become worn or have holes, ask the Program Manager for a replacement. If mattress covers are removed, residents will be required to pay for replacement, and may also be required to pay for a replacement mattress.



Do not store food in your cottage except for non-perishable items in pest-proof containers.



If you have guests who mention that they have bedbugs in their homes, they are likely to have carried them into your cottage in their clothing. If this happens, ask the Program Manager for information on how to identify bedbugs, and for instruction on what to do if you have them.



New residents will be asked to wash and dry all their clothing and bedding before moving in if they report they have been living in a place with bedbugs, and other belongings may require additional treatment.

Service Animal and Pet Policy Residents are not allowed to own pets unless they are service animals. Visitors and volunteers may bring pets into the Village during visiting hours if the visiting pet is not disruptive. Residents are required to have a letter from a physician to verify that an animal is their service animal. Service animals are subject to strict rules regarding licensing, spaying/neutering, inoculations, sanitation, standards of care and behavior, and documentation of these requirements.

RENTAL AGREEMENT ATTACHMENT 3 Keys

MAILBOX: Your mailbox is located the front hallway of the House. Your cottage number is on the outside of the box. Outgoing mail can be put in the locked outgoing mailbox. KEYS: You are given 1 key to your mailbox. A duplicate key is kept locked on site and is only used when requested by resident for lost or misplaced keys. Keys are not to be duplicated for any reason, except by Management during normal business hours 9:00 am to 5:00 pm Monday through Friday. There is a service charge for each key duplicated, or not returned on termination of occupancy. See the Program Manager if you need a new key. DELIVERY OF PACKAGES: Packages can be picked up at the office during office hours.
