Qualifying Events Your benefit elections are irrevocable for the 2015 Plan Year until the next open enrollment period unless you experience a Change in Status as defined under the Regulations by Department of The Treasury IRS § 125. All changes must be made within 30 days from the date of the qualifying event. Examples of change in status qualifying life events include:  Change in legal marital status, including marriage, death of a spouse, divorce and annulment.  A change in the number of dependents, including birth, death, adoption and placement for adoption.  A change in employment status of the employee, spouse or dependent including termination or commencement of employment, part-time and full-time.  A dependent ceasing to satisfy eligibility requirements for coverage due to attainment of age.  A change in place of residence of the employee, spouse or dependent.  Significant cost of coverage changes (including coverage curtailment and the addition of a benefit package).  Change of custody, judgments, decrees or court orders.  A change in coverage of a spouse or dependent under another employer’s plan.  A COBRA qualifying event.  Loss of coverage under the group health plan.  Become entitled to Medicare or Medi-Cal. If you divorce and fail to remove your former spouse as a dependent on your plans, or leave an ineligible child on your coverage, it is considered insurance fraud because you have an ineligible dependent on your insurance contract.
