Procedures for an Employee Needlestick 1. Wound, and skin sites that have been exposed to blood or body fluids should be washed with soap and water. Mucous membranes should be flushed with water. 2. Notify your supervisor immediately of the exposure incident and complete an incident report. 3. Collect the source individual’s blood with the patient’s consent to test for HIV, Hepatitis B (HBV), and Hepatitis C (HCV) infectivity. The tests to order are HBsAg, anti-HCV, and rapid HIV antibody. Also order HIV antibody testing to be sent to the State Lab. 4. Send source patient’s blood to Ashe Memorial Hospital for the rapid HIV testing. Have the patient sign the AMH HIV consent form. Submit the order request for rapid HIV test, the consent form and the drawn blood in a serum separator tube to the AMH lab. 5. HCV testing will be sent to LabCorp. 6. HBV testing will be sent to the State Lab. Check Hepatitis B Screen and “Source patient from whom exposure occurred” on front of lab form. 7. If consent is not given, notify the Nursing Director to obtain consent by the Health Director through the legal system. 8. When the source individual is already known to be infected with HBV,HCV or HIV, testing for the source individual’s known HBV, HCV or HIV status need not be repeated. 9. The exposed employee’s blood will be collected with the employee’s consent and tested for HBV, HCV, and HIV to determine infections status and immune response of HBV vaccine at time of exposure. 10.If the employee does not give consent for HIV serological testing during collection of blood for baseline testing, preserve the baseline blood sample for at least 90 days; if the exposed employee elects to have the baseline sample tested during the waiting period, perform testing as soon as feasible. 11.The tests to order for the exposed employee are anti-HBs, anti-HCV and HIV antibody. 12.Send the HCV test to LabCorp. 13.Send the Hepatitis B test to the State lab. Check Hepatitis B Monitor and “Previously vaccinated health department employee with percutaneous exposure to Hepatitis B infection”. 14.Send HIV test to the State Lab. 15.All orders for lab work and HBIG and/or vaccine will be obtained from the Appalachian District Health Department Medical Director. Follow-UP 1. If source is HCV positive, repeat anti-HCV testing in 6 months for the employee if the employee’s baseline anti-HCV is negative. 2. See Table on p. 18 of Bloodborne Pathogen Manual for Hepatitis B follow-up. 3. Follow-up testing for an employee exposed to a known HIV positive source will be done at 6 weeks, 12 weeks and 6 months after exposure. 4. If the employee becomes infected with HCV after exposure to a source co-infected with HIV and HCV, extend HIV follow-up testing to 12 months after exposure as well.
