Position Announcement: Apply by Monday, February 20, 2017 Position: Population Health Coordinator, Delaware County. The Population Health Coordinator, Delaware County position is a salaried, exempt position and not subject to the provisions of the Fair Labor Standards Act. Reports to: Population Health Coordinator, Tioga County General Duties and Responsibilities: The Population Health Coordinator, Delaware County is responsible for implementing the health disparities component of the Southern Tier Population Health Improvement Program (ST PHIP) in Delaware County. The Population Health Coordinator, Delaware County will work with the HealthlinkNY Community Network, ST PHIP, and other county-level Population Health Coordinators to help develop and implement a regional strategy to address health disparities in the Southern Tier of New York State. Specific Responsibilities: The responsibilities of the Population Health Coordinator, Delaware County include, but are not limited to, the following: 1. Utilize the ST PHIP inventory/assessment of health disparities and other available data, reports, and information to inform and address health disparities in Delaware County. 2. Implement ST PHIP work plans to address health disparities and work collaboratively to implement recommendations for addressing health disparities in Delaware County and the ST PHIP service area. Consider how implementation of ST PHIP work plans can add value to the Delaware County Community Health Improvement Plan, Community Services Plans, and other planning activities or projects that align with the New York State Prevention Agenda and/or are implementing interventions to address health disparities. 3. Participate in the ST PHIP meetings, work groups, and forums as required. 4. Represent ST PHIP and the Rural Health Network of SCNY at appropriate meetings, work groups, and forums in Delaware County. 5. Design and coordinate stakeholder engagement activities across programs to better understand and address health disparities. 6. Provide support of evaluation needs across programs, including evaluation design, data mining, and reporting. 7. Collaborate with elected officials that participate in regional planning for health-related areas to become knowledgeable about priorities and seek common goals. 8. Actively cooperate and collaborate with Southern Tier PHIP – HealthlinkNY Community Network staff. 9. Maintain knowledge of developments and trends in the field of population health, specifically social determinants of health and health disparities. 10. Serve as a member of the RHNSCNY staff team and seek ways to continuously improve and support the work the RHNSCNY. Seek tangible, effective ways to integrate and apply all RHNSCNY services to provide the maximum benefit to those served. Minimum Qualifications: Bachelor’s Degree in public health, public administration, planning, human services, or related field and three years related experience, or an acceptable combination of education and work experience.
