FAN BUS PERMISSION FORM Event: State Football Quarter-finals Date: Saturday, November 11th Site: Alexandria High School I understand that there will be a fan bus (CHARTER BUS) to the state quarter-final football game on Saturday, November 11 in Alexandria. I hereby give permission for my son/daughter to ride the fan bus to Alexandria on Saturday, November 11th. The bus will leave at noon from the Athletic Center parking lot at the High School. Cost is $15 for the bus. Admission is $9 for adults and $6 for students The fan bus is open to students in grades 7-12 and adults. Any students 6th grade or younger must have a parent accompany them. Please return this form with payment to the High School Principal’s office. Make checks out to ISD #186 Estimated return time is 8:00pm. Deadline to sign up is Noon on Thursday, November 9 at 3:00 pm. *ANY STUDENT RIDING THE BUS MUST HAVE PARENTAL PERMISSION *BE ADVISED THAT IT IS DIFFICULT TO PREDICT THE RETURN TIME, BASED ON THE FORMAT OF THE COMPETITION *NORMAL BUS SAFETY RULES MUST BE FOLLOWED. *SCHOOL RULES MUST BE FOLLOWED AND MISBEHAVIOR WILL BE DEALT WITH USING NORMAL SCHOOL CONSEQUENCES. *EACH RIDER MUST LISTEN CAREFULLY TO THE CHAPERONE AND MUST ARRIVE AT THE POST GAME MEETING SPOT ON TIME. *THE CHAPERONE WILL REPORT ANY PROBLEMS WITH STUDENTS TO THE ATHLETIC DIRECTOR AT THE GAME OR THE FOLLOWING DAY AT SCHOOL. *ANYBODY RIDING THE BUS TO THE GAME MUST RIDE BACK HOME ON THE BUS. NO EXCEPTIONS. *REMEMBER THAT YOU ARE REPRESENTING YOUR SCHOOL AND COMMUNITY ON THIS TRIP. PLEASE ACT ACCORDINGLY. *HAVE FUN!
