Parental Consent Form for ImPACT Baseline Testing STMA High School

I have read the attached informational letter and understand its contents. I understand that my child’s involvement in the ImPACT™ concussion baseline testing is highly recommended but not required for participation in athletics at STMA High School. I give permission for my child to receive ImPACT concussion management baseline testing. I understand there is no cost for this baseline test. To STMA High School Parents: The safety and well-being of student athletes at STMA High School is of utmost importance. Concussions (mild traumatic brain injuries) are injuries that athletes may suffer in almost any sport, and it is vital that they are managed properly. The greatest risk following concussion is a second concussion before the first one has adequately healed. This can trigger a life-threatening condition called ‘second impact syndrome’. Free concussion management program STMA High School, in partnership with Sister Kenny Sports & Physical Therapy Center STMA and the Buffalo Hospital Foundation, is pleased to offer a new concussion management program called ImPACTTM for all student athletes in grades 9-12. ImPACTTM is a computer-based program that measures brain function in areas such as: attention span, memory, and reaction time. The ImPACTTM test is administered before athletes participate in a sport to get a baseline score. If a head injury occurs during the course of a sports season, the same test is repeated. The ‘before and after’ comparison assesses the degree of any brain injury or impairment and is used to help determine when it is safe for an athlete to return to playing their sport. Baseline testing for STMA High School athletes is optional, free, and will not in any way affect team selection or your child’s participation. If your child sustains a concussion during the school year, post-injury testing is available at Sister Kenny Sports & Physical Therapy Center. There is no charge for post-injury testing, but your child will need to be cleared by an approved Health Care Provider, and you may have a charge depending on your insurance coverage and who you see. Health Care Provider clearance after concussion is required by the Minnesota State High School League, and the ImPACTTM test along with our return to play protocol will give the clearing provider more information to make a decision. Baseline testing Baseline testing for STMA athletes will begin this summer, starting with athletes who participate in fall sports. Attached to this letter is a consent form for your student athlete to receive baseline testing. If you want your child to be tested, please complete the consent form and send it with your child when they are baseline tested, or it can be turned in at the Activities Office. We are grateful to the Buffalo Hospital Foundation for their fundraising efforts that brought this program to STMA High School and made it possible to provide baseline testing at no cost to athletes. Testing is to begin on August 8th in the Computer Lab. Additional information about the ImPACTTM program is available on the Buffalo Hospital web site (www.buffalohospital.org). You are also welcome to talk to Nathan Koep at (612) 247-0932 or [email protected] Sincerely, Brian Benson Activities Director
