MEMORANDUM OF AGREEMENT • This Memorandum of Agreement ("AGREEMENT "), to be effective as of the 20 111 day of May, 2017 is made and entered into by ROTARY DlSTIU CT 3780, as represented by its Distri ct Governor, CHLTO BORROMEO (hereinafter referred to as FlRST PARTY), QUEZON CITY DIVI SION OF SCHOOLS, as rep re sented by Schools Division Superintendent, DR. ELIZA BETH E. QU ESADA (hereina!ter referred to as SECOND PA RTY ), and th e DEPA RTMENT OF HEALT I-l, as represented by NCR Director ARIE L VALENC IA (hereina!ier referred to as THlR D PARTY), collectively referred to as the PA RTIES whose signatures appear heretmder . WITNESSETH: That WHEREAS, The PARTIES agree to come together to undertake an official cooperation whereby public school s in Quezon City become recipients of dynamic education-related services that the PARTIES jointly assume. NO W, THEREFORE, for and in consideration of the foregoing premise, the PARTIES hereby agree, jointly and severally to take on the following respon sibilities: A. FIRST PARTY shall :

Assign its constituent Rotaty clubs a school/s to adopt on the basis of the school's proximity ; 2. Ensure the equitable adoption of schools in tenus of distribution ; 3. Provide logistic supp01i for the successful conduct of the program ; 4. Supply manpower to cany out the objectives of the program; 5. Be the principal implementer of the program ; 6. Monitor the progress of the program and advise stakeholders via the submission of status report B. SECOND PARTY shall: 1.
The SECOND PARTY shall closely coordinate with the FIRST PARTY on the adoption process and recommend measures to facilitate equitable adoption ; 2. IdentifY school-participants based on need; 3. Suppmt the program with personal and material assistance; 4. Provide data and documents that serve as basis for specitlc comses of action; 5. Coordinate with other entities for the success of the program C. THIRD PARTY shall:

The THIRD PARTY, in close coordination with the FIRST PARTY, shall bring to the program health-related measures that promote the general well-being of the studentTy; 2. Provide essential medicines;

Jointly, the PARTIES shall undettake the following services in order to accomplish the purpose ofthis AGREEMENT: I.

Brigada Eskwela - Clean-up drive of schools occuning annually before statt of classes. It includes repainting prqjects of schools whose aesthetic attribute has faded. Araw ng Kabataan - Assembly of the youth wherein their achievements in their academic performance as well as theiJ extra-curricular feats are recognized. End TB Program -Medical outTeach program in schools to put an end to TB. It seeks to bolster awareness among students in order to keep the disease at bay. Milk Feeding Program - Regular nutrition program aimed at promoting the drinking of milk runong elementaty students. It seeks to eliminate undernourishment among the children. Water Sanitation & Hygiene (WASH) Program - A program aimed at addressing the basic sanitation problems of the school whereby the Rotmy will undettake to provide suppmt in improving tl1e present sanitation infrastructure of the school and to hold seminars and/or distribute materials to teach proper hygiene to the students. Leadership Training Seminar - Rotary will conduct training programs whereby students' leadership skills are honed. Through this seminar, students peek through their potentials as leaders of different organizations. Literacy Campaign Program - An educational program that reinforces students' literacy thJOugh enl1anced cognitive and practical drills SPED - A special educational program iliat seeks the integration of special children into the mainstream learning environment. The Rotmy shall tap its resources to set down the mechanism. Rotaract - A segment of the Rotaty composed of college students empowered to implement their own programs that benefit the community Interact - A segment of ilie Rotary composed of high school students empowered to implement their own programs that benefit tl1e community Anti-Dengue Activities - An undertaking of the Rotary featuring fogging and misting activities in schools and information dissemination to guide the students on the proper clem1-up drive so as to eradicate dengue. Spotts Development Program - An undertaking of the Rotmy that seeks to sponsor and support promising athletes from schools in order to reach their full potential achieve international recognition
The projects are not limited to ilie aforeme11tioned. Both patties can initiate and propose programs as deemed necessmy to achieve a vibrant and dynamic education-related services. This AGREEMENT takes effect immediately upon signing by the PARTIES hereto, together with their respective witnesses and shall remain effective and valid until the PARTIES have fully complied with the responsibilities in this AGREEMENT.
