GENERAL POLICY INFORMATION AND WAIVER REGISTRATION Advance registration for Mather Academy courses is strongly recommended to secure a student’s place in class and to ensure that the class will go forward. Enrollment is limited and students are accepted on a first-paid basis. Registrations will not be processed without payment. When registering a student under the age of 18, the Minor Participant Information Form must be submitted. Registration continues to the date of the first class unless a deadline is specified or until class is filled. Individuals may register in the following ways: Online at Beaufortartscouncil.org ● In person at the Mather Academy, Beaufort Arts Council ● By telephone at 843.379.2787 ●
Successful processing of the registration form and full payment is necessary to guarantee enrollment. Please be aware that incomplete information on registration forms may delay successful processing and enrollment. Class Size (minimum and maximum) is predetermined by instructor to give students the ultimate learning experience and individual attention. Only fully paid registrations will be considered as enrolled. You will receive a confirmation upon completion of reservation. Class Levels Unless otherwise noted, classes are open to all and recommended for beginner to intermediate levels. Children should be enrolled within their age groups. Teenagers (16+) are welcome to join adult classes with parent consent. Only individuals 21 years of age and older are permitted to register for classes that include alcoholic beverages. Materials are included in price of all classes, workshops, and programs, except where noted in the class description. Participants are expected to provide their own smocks or wear protective clothing. Adults (18 or older) use hazardous supplies at their own risk, and are encouraged to read product labels and to ask instructors for alternatives if necessary. The Mather Academy instructors for children’s art classes generally supply only safe and age-appropriate materials, as dictated by industry standards. Officemgr/MatherAcademy/FallClasses2015

PAYMENT Full payment is required at the time of registration in any of the following ways: ● Credit card (Master Card, Visa, or American Express) ● Checks, c ash or money orders (payable to Beaufort Arts Council) will only be allowed for mail-in registration or in-person registrations. CANCELLATIONS and REFUNDS Mather Academy reserves the right to withdraw classes due to inclement weather or unexpected circumstances. In the event that a class is withdrawn, notifications to students will be made one (1) business day prior to the start of the class. Full refunds or credits will be given when Mather Academy cancels a class. Individuals who miss class do so at their own loss. Class cancellations will be listed in the following ways: ● Posted on the Mather Academy/Beaufort Arts Council website @ beaufortartscouncil.org ● Emailed n otification sent to registrants. Please note that payments are non-refundable if withdrawal is made by participant an administrative fee will be assessed. Students will be allowed a ½ credit that at the discretion of the instructor may be applied to future classes. Credits are not transferable. Notification of intent to cancel must be received no less than one (5) business days before the scheduled start date. WAIVER As a participant, or parent or guardian of a participant, permission is granted to participate in the Mather Academy programs listed above. Participants understand and agree that they may be photographed and/or videotaped for the promotion of Mather Academy programs. I understand that there are risks of physical injury to the participant(s). Considering all possible risks, on behalf of the participant(s) and myself, I voluntarily waive, release, discharge, and hold harmless the Mather Academy its employees, supervisors, appointed officials, agents, representatives and volunteers from all claims for all injuries to participant(s), no matter how severe. Furthermore, I give consent for emergency medical treatment to the participant(s), no matter how severe. This waiver does not extend to any such claim or liability that is caused solely and exclusively by the gross negligence of the Mather Academy or its employees, supervisors, appointed officials, agents, representatives and volunteers.
