RISK ASSESSMENT/ MANAGEMENT FORM

To be completed before any trials, competitions or gala days SPORT: OPEN BOYS and YEAR 7-10 BOYS FOOTBALL KNOCKOUT DATE: WEDNESDAY 13 JUNE (BACK-UP WEDNESDAY 20 JUNE), 2018 VENUE: JUDY MASTERS PARK, BALGOWNIE EVENT CONVENER: DEAN MEROLA, HOLY SPIRIT COLLEGE, BELLAMBI MOBILE CONTACT: 0404 487 226 PROPOSED NUMBERS: 224 participating students (max) - with a total of 12 teams, maximum of 14 players per team. 10-15 Supervisors - based on one adult supervisor per team Supported by external referees 1 First aid official GENERAL INFORMATION/COMMENTS All teams will be informed of draws prior to the day along with expectation. Schools will have received information about the rules and expectations prior to the day. The coach/supervisor of the team should monitor player safety and remove any player at risk from the field of play. Any player who is over aggressive or partaking in rough play should be removed from play and issued with a warning. A repeat performance should result in the player being suspended for the remainder of the event. VENUE MAP: Sec Football KO - Balgownie.doc

Posts. Fields are maintained by Wollongong City Council and Balgownie Soccer Club Qualified first aid attendants to be in attendance at both venues with related medical supplies and ice. Council and Soccer Club maintained facilities Toilets checked for cleanliness, toilet paper, lighting etc: TAPS – access for hydration etc. Access to Judy Masters Park grounds via Para Street. Car parking is available in at the end of Para street adjacent to the fields.
