Criminal Records Check / Finger Printing   Instructions   

1. Come to the AMAESD Office to pick up and fill out a LiveScan fingerprint request  form.     2. Call to setup an appointment at one of the following locations.   a. Alpena County Sheriff’s Department  i. Phone Number : 989.354.9839  ii. Office Hours: Monday‐Friday 10:00 AM to 4:00 PM  iii. Applicants must have the LiveScan Fingerprint Request Form  completed to give to the Sherriff’s Department.   b. The UPS Store  i. Phone Number: 989.356.8989  ii. Applicants must have the LiveScan Fingerprint Request Form  completed to give to UPS Store employee.      3. Return LiveScan form to Fingerprinting Information Office once you have been  fingerprinted. (You will not be able to work in any school within the AMAESD   unit this form is returned)   If you are a PCMI Employee, please read the following information.   1. PCMI will not start your second phase of paperwork until l the results of your  LiveScan have been received by the AMAESD.   2. PCMI will be notified by the AMAESD once you are accepted.  3. PCMI will contact you to complete the second phase of paperwork  4. Once PCMI has processed all of your paperwork, you and AMAESD will be  notified via e‐mail that you are in compliance.
