[email protected]) within two weeks from your participation at IPDPS 2014: (a) either the W-8 (foreign) or W-9 (US residents only) completed, physically signed and then scanned, (b) receipts for your travel, (c) a statement from your research advisor or the department head certifying that you are not being reimbursed from somewhere else for the travel expenses covered by this grant, and (d) address where the reimbursement check is to be mailed.
