Rankone online and medical information Before a student participates in any sport including conditioning and practices, that student/athlete must provide the following information to the respective coach. 2. Forms must be accessed online at corpuschristiisd.rankonesport.com: 1. UIL health questionnaire and/or physical examination form with student, parent, and physician’s signature. Must be turned into the respective coach. Physicals are good for 2 years unless, physician requires yearly physical. 2. UIL Acknowledgment of Rules 3. Emergency Card 4. MEDICAL RELEASE TO PARTICIPATE AGAIN 1. Injuries-If an athlete is injured and is seen by a doctor, he/she must have a written doctor’s release before he/she can practice or compete in athletic contests. A doctor’s written release must be presented to the coach/trainer prior to the practice or the contest. 2. Athletic Class Parent note: A parent’s note excusing their athlete from workouts will be honored for three school days only, after that a doctors not is needed to be excused. However, as in the classroom that student will make up the workouts that he/she missed the previous days. 3. Paperwork- When an injury happens at practice or contest, be sure to collect all paperwork from the coach of that sport, concussion forms, CCISD insurance (may or may not pay) etc. 4. Concussion- If a concussion is suspected, we recommend you take your athlete to your family doctor to be examined, and we will also contact our athletic trainer to examine your athlete. If it is determined that the athlete has a concussion, by CCISD policy that athlete will begin a five day concussion protocol after cleared by the doctor. This can only begin when the athlete has not experienced symptoms for 24 hours. If during any time during the protocol they experience symptoms, the 24 hours will start over and so will the concussion protocol.
Policies and Expectations of all Student Athletes It is a PRIVILEGE and it must be EARNED to play for our program, nothing will just be GIVEN. 1. Grades – Don’t pass, don’t play. This is a UIL rule for the State of Texas, all students must obtain a 70% and above grade point average at the end of each six weeks to be eligible to compete in an athletic sport. If your student/athlete is taking Geometry and that student gets a GPA of 60-69% they are still allowed to participate because it is an advanced class. 2. Classroom Attitude- Remember you are a STUDENT FIRST before an athlete. A. Respect- If you cannot respect your principals, teachers and staff’s authority, what makes our coaches believe that you will respect our authority? B. Discipline- Be where you are supposed to be, when you are supposed to be, doing what you are supposed to be doing ALL THE TIME!! C. Commitment- Be committed to the TEAM’S success that means in the classroom too. Follow the program’s vison both inside and outside the classroom. 1. Faith, 2. Family, 3. Education 4. Sports 5. Misc. D. Work Ethic- We must be willing to outwork every opponent and that’s in the classroom too. Everyone wants to be successful in life but few are willing to do what it takes, that is on and off the field. Work every day as if your job depended on it, because in the real world it will. 3. Classroom discipline actions – If a student/athlete has been given a consequence for his/her action in the classroom or school facilities from a teacher or administrator: A. Teacher emails/infraction log: If the head coach is receiving complaints of that athlete through emails or infraction logs, the consequences will be
determined by the head coach of that sport or coach of that athletic class. Loss of playing time, workout discipline, loss of athletics class etc… B. Athletic class: If an athlete is not prepared for athletics class (dressing out) that student will still workout but a separate workout from the class. Coaches will document all times that the athlete did not dress out and what the consequences are. If the student does not dress out in excess of t hree times, the student may be removed from athletics class and placed in another elective. C. Detention: Consequences determined by the head coach of the sport, multiple detentions can lead to loss of playing time, removal from athletic class, work out discipline, or removal from the team. D. ISS: If a student athlete receives ISS the week of a game, that student will lose playing time or may be benched for the game plus an extra workout at practice or athletics class (determined by the HC). If ISS is given on game day, that athlete will be benched. More than three trips to ISS that athlete will be removed from the team and athletics class. Depending on the severity of the infraction this could lead to removal from the team, removal from athletic class and program. E. Suspension: Any student athlete that is suspended from school will automatically be removed from that sport, athletic class and cannot participate in the next available sport for the school year. F. SSC: If a student athlete is sent to SSC, that athlete will automatically not be able to participate in Kaffie athletics class or sports. When a student athlete is removed from the program that decision will come from the Athletic Coordinator and Head Coach of that sport. If a student athlete is removed from a sport because of discipline problems, he/she will not be eligible to participate in the next sport. If decided by the Athletic Coordinator that the athlete will be given another chance after multiple conduct issues, there will be an athlete contract that will be signed by the AC, HC, athlete, parent and principal. Athlete must follow all guidelines for that contract.

Major discipline actions- These major infractions will not be tolerated in our program and your student athlete will be removed from our program and possibly for the rest of their middle school sports at Kaffie. A. Bullying- Whether it be physical or cyber B. Fighting- Head coach will investigate before decision is made. C. Theft- Whether you take it or you are caught with the person stealing it. D. Social Media- Any type of negativity towards a school, person, sexting etc. E. Vandalism- To our school property, fellow classmate property, teacher property etc. F. Drug and Alcohol use- This includes extra-curricular activities. G. School Officer citation
Team Expectations 1. Practice: Each athlete is expected to be at every practice and on time. Being late either means you are not reliable, or something else is more important than your team. I f your athlete has

more than 3 unexcused absences or multiple tardiness, that athlete may be removed from the team and will not be eligible for the next sport. Please understand each situation will be handled at the coach’s discretion. A. Excused absence: Death in the family, ill (with a doctor’s excuse), tutorial’s with a teachers note, extracurricular SCHOOL ACTIVITIES B. Unexcused absence: Hand written notes from mom/dad or guardian (exception at the discretion of coaches) vacation, select/club team events etc. Consequences: A. Tardiness: Extra work out to be performed before or after practice at the coach’s discretion. If tardiness becomes an issue, that athlete will lose playing time and could lose their starting position. All decisions will be made at the head coach’s discretion. B. Unexcused absence: Extra work out to be performed before or after practice. If the athlete misses practice before game day, that athlete will be benched for that game. If the athlete misses practice the week of a game, that athlete will lose playing time and could lose their starting position. All decisions will be made at the head coach’s discretion. Reminder, 3 or more unexcused absences will get the athlete removed from the team and that athlete will not be able to try out for the next available sport. Game Day: All athletes must be in the locker room at that head coaches requested time. Remember it’s an all week process and morning of game day, from the classroom, to the locker room after school. We are on a strict time schedule, when the bus arrives, we must load up as soon as the bus gets here and leave to the field/court. Game Day apparel: All athletes will have game day apparel that will be approved by our principal. Each coach will have assigned game day apparel for that sport. Consequences: A. Tardiness: Extra work out to be performed at the next practice. Athlete can be sent home, can lose playing time and starting position. If you miss the bus, you will not play that game and the next scheduled game. B. Game Day apparel: Those who are not in game day apparel will not be allowed on the bus to travel to the game. If you need a tie, I got plenty to go around.
6. Team Behavior: All athletes will be held accountable for their actions on and off the field. All athletes are expected to play with class but still compete at a high level. This includes games, referees, team activities (MS night, volunteer work etc.) and other team facilities. Remember you are not only representing yourself but your team, your school, coaches and parents/guardians. Someone is always watching and listening. 7. Club/Select Sports: If your son/daughter plays in a club sport and it is going to interfere with school sports and you know ahead of time, please do not tryout, we need 100% commitment from each athlete. If you can schedule your club/select team around school sports schedule that is great. Please remember, school sports come first before club/select sports.
Season Completion
The student-athlete will have an opportunity to earn a spot on a team. The student-athlete will have a two week period to determine if they want to proceed and complete the season in the specific sport they start. If within these two weeks the student decides he or she does not want to complete the season, they can notify the head coach of that sport without penalty, and will be eligible to try out for the next sport. However, if the student-athlete quits after the two week period, they WILL NOT be eligible to participate in the following sport, but may return to the next sport. Ex. Volleyball: Student-athlete quits after 2 week grace period Basketball: Student-athlete is ineligible to participate Track/Swimming/Golf/etc.: Student-athlete regains eligibility to participate.
As educators we encourage all students to finish the task in which they start. This will help develop healthy work habits that will carry over into the real world after high school.
Parents Expectations 1. Behavior: We encourage every student-athletes parent(s)/guardian(s) to attend every game of the season for that sport that is in season. We ask that you encourage and cheer on our student-athletes and coaches in a positive way. We also ask that you have good sportsmanship towards the opposing teams, their fans and officials. Please DO NOT approach our coaches for discussion about: Playing time, team strategy, play calling. If you have a concern about an issue other than these, feel free to schedule a conference the day A FTER the game. We are willing to answer questions about your athlete only. 2. Method for Resolving Concerns: If a parent has a concern about an issue, consequence that was handed down etc, there is a chain of command you m ust follow. First there will be a conference with the head coach to discuss the issue. If there is still a concern after, there will be a conference with the head coach and Athletic Coordinator. After that if the issue is still not resolved there will be a conference with the principal, Athletic Coordinator and head coach. 3. Pick up from practices and games: Parents/Guardians are expected to pick up your athlete from practice and games on time. We will not leave your child unsupervised, however, if this becomes a problem we will notify the principal and this could lead to removal from the team. If a student is left for multiple hours we are by law to contact the police. 4. Travel to athletic contest: All team members MUST travel to the contest by means of transportation provided by the district. 5. Travel from athletic contest: If an athlete is going to go home after the contest is completed with their parent/guardian, they must have a parent release form filled out and signed by a parent, administrator and head coach, before and after school and at least 24 HOURS PRIOR TO THE EVENT. No exceptions will be made after that 24 hour grace period.
If a parent is removed from a game and their behavior is unacceptable, that could lead to being ban from ALL CCISD contest for the entire school year at every level. CARE OF SCHOOL EQUIPMENT
The athlete is responsible for all equipment issued to him/her. The athlete or his/her parents must pay for stolen/lost or damaged equipment. This is from uniforms all the way to locks for lockers in athletics class. This becomes a discipline issue and will be handled as a student code of conduct violation.
Athletics Class Each student is equally given the opportunity to EARN a spot in our athletics class. This spot must be earned throughout the entire 6th grade year and 7th grade year. Whether you are th in PE or Team Dolphin your 6 grade year or PE your 7th grade year you have a chance to EARN a spot in our athletic class. If you were in 7th grade athletics this year, you must prove to us that you deserve a spot back in athletics for next year. Criteria that we are looking for in individuals that are attempting to EARN a spot in athletics class, is to follow the four program principles consisting of 1. Respect 2. Discipline 3. Commitment and 4. Work ethic. All of these are in and out of the classroom from having grades to keep you UIL eligible to play sports, to behavior in and out of the classroom. The most important, DO YOUR JOB for the success of the team on and off the field. We also ask that your athlete attempts a fall and spring sport, this will give that athlete an opportunity to be exposed to multiple sports. Understand that this is not the “running club” or advanced PE, we have high standards and expectations for each athlete in the classroom and athletics class. If they are not meeting our standards (conduct, grades etc.) in the classroom, there will be a consequence in athletics class whether you are in season or not for that sport. We are trying to build a program that will prepare each athlete for the next level in high school, and to become productive citizens and future leaders. Top athlete of the year & Hustle and Heart Award Each year the coaches will decide who gets the Top athlete of the year and Hustle and Heart Award. To be nominated for the top athlete of the year award, he/she must participate and excel in 3 sports, be a student/athlete, student first athlete always second. When in the classroom he must give 100%, pass all classes, be respectful to teachers, staff and peers. The student/athlete must have great conduct in the classroom, have discipline in the classroom and willing to help others when needed. On the field of play, the student/athlete must give 100% to his team’s success, not be selfish, respects his teammates and coaches, has good sportsmanship and represents his team on and off the field in a positive manner. To be nominated for the Hustle and Heart award this athlete may not have made the A team or played much, however, continued to play hard, practice hard and never complained about the situation. Gave it all they had in the classroom and in athletics class and no matter what the task at hand was, gave it 100%. Every opportunity they were given they did it with hustle and heart. For both awards there can be multiple athletes to receive this award for both girls and boys.
