Follow instructions to download and print a PDF copy of your personalized membership application

Complete the hard copy paper Application Form

Obtain US$ DD from your bank by submitting: 1. Above Application form

Upload scanned "Certifying Letter/s" obtained from your Company Enter credit card details and make the payment. Membership Fee : MDRT US$ 550, COT-US$ 600, TOT-US$ 1100

2. Copy of your certifying letter 3. Covering request letter as attached Membership Fee for MDRT - US$ 550, COT -US$ 600 and TOT - US$ 1100

Enter payment details on the application form Once payment confirmed, registration is complete. Print the application form and payment receipt for your records. Approved membership confirmation will be sent to you via email

Courier the completed Application form, US$ DD and Certifying letter before March 1st, 2014 to : Membership Department, 325, West Touhy Avenue, Park Ridge, IL 60068, USA
