The staff at Cottonwood Middle School would like to welcome you to the 2013-2014 school year. We are excited about the upcoming school year and the opportunity to work collaboratively with you. Each school year brings about many changes. We want to assure you that your child will receive excellent opportunities for growth.
Students must use their agenda as a “Pass” to go to the library, or to go from room to room at times other than normal changing of classes. If a student loses his/her agenda, he/she will be required to purchase a new agenda at the price of $5.

Rest assured that this is a ‘normal behavioral pattern’ and we are available to assist you through these times. Pre-adolescent students are very capable of respectful and responsible thought and action in their daily routines. Some students will require a great deal of support while others will need very little guidance.

Vision & Mission Statements
All students participating in district extracurricular or interscholastic athletics will be assessed a $30 minimum participation fee, except for football, which is a $50 fee. Participation in extracurricular activities is based upon cooperation, good sportsmanship and behavior, responsibility and academic eligibility.
“Community of Learners Motivated to Succeed” Cottonwood Middle School provides each student a challenging education in a safe, supportive environment that promotes self-sufficiency in a community of life long learners motivated to succeed.
Grades will be checked on a cumulative basis at the end of each three (3) week period. If a student has one (1), or more failing grades and/or two (2), or more D’s; the student will be deemed ineligible for a minimum of three (3) weeks until the next three week grade check. Upon improvement demonstrating he/she is passing on a cumulative basis, the student shall be reinstated to the team(s) or extracurricular activities until a subsequent check is performed. A student may also be deemed ineligible for other reasons than academic performance. If a student is absent or removed from school for reasons other than a specific prearranged appointment, he/she is NOT allowed to participate in any school activities on that day. This includes, but is not limited to dances, extracurricular activities and field trips, etc.
Promotion/Retention: Students are expected to make passing grades in their courses (D or higher). Students who do not successfully complete their classes will be considered candidates for retention. A student must pass three of the four quarters in each of the core subjects in order to be promoted to the next grade level.
Dance Eligibility: Prior to each school dance, student eligibility will be checked. Students with a failing grade (F) will not be eligible to attend.
National Junior Honor Society (NJHS): NJHS is a national organization open to CMS students who meet the organization’s guidelines. Membership is open to students in grades 6-8. Students may apply for membership in January of their 6th grade year. Inductions are held in September and January each year. Selection is based on five criteria: citizenship, service, leadership, scholarship and character. Members must maintain a high grade point average (the national minimum is a 3.0) and become good citizens (by participating in service projects). The NJHS requires school service to the community, school, or other organizations. These projects help students meet the required yearly service hours. The CMS chapter has a limit of 25 members per year.
Homework: Most classes will assign homework on a regular basis. Most students will have between 1 to 1 1/2 hours of homework each school night. Homework is a portion of the class grade. Homework is due when indicated. Full credit is not to be expected for late work. Honor Roll: Students with A’s and B’s for the quarter will be listed on the honor roll. Students with all A’s will be listed with highest distinction, Principal’s Honor Roll. Make-Up Work: When a student is absent, it is his/her responsibility to ask the teacher for make-up work and the acceptable timeline for completion. Students shall have at least one day for each day of excused absence. Long-term projects and previously assigned work that was due upon the day the student was absent, will be due upon the student’s return.
Clubs and Activities
Parent Conference: Each year the district schedules two formal conferences for parents. Your child should be present at these conferences. However, parents may request a conference anytime throughout the school year by contacting the office. The teacher will return your call to schedule an appointment with you. Report Cards: Are sent home at the conclusion of each quarter (9 weeks). Quarter grades are used to decide promotion/retention and honor roll status. Progress Reports: Are sent home at the designated midpoint of the quarter, which is every 4 1/2 weeks. Parents can access Family Link via the District website at www.cocsd.k12.az.us click on the ‘Parent’ tab at the top of the page. To access Family Link, please call the CMS Front Office 634-2231 to obtain your child’s PIN Number and Student ID Number. This will allow you to monitor your child’s academic

GENERAL INFORMATION Arrival & Dismissal Times: Students are to arrive on campus for the beginning of school NO EARLIER than 8:00 a.m. This is for their safety as there is no supervision until this time. The cafeteria opens at 8:00 a.m. If your child must be dropped off early; they are to report immediately to the cafeteria. The school day 3
begins at 8:35 a.m. ends at 3:35 p.m. Every Wednesday students will be dismissed 2:25 p.m., to allow teachers to participate in professional learning communities/trainings.
15 days, a student may be placed on a truancy contract. If a student continues to be habitually absent, the parents and the student maybe cited to the juvenile court system.
Attendance: Attendance is vital to academic success. Adherence to the school attendance policy is the responsibility of parents and students. Unauthorized absence will not be taken lightly.
Leaving School Early: Parents/guardians must sign their child out in the office. The office secretary will call your child to the office for you. Please do not go to the classroom to pick up your child. Teachers are NOT allowed to release students without a call from the office. This is for your child’s safety.
District Attendance Policy: All students sixteen years old or younger are required by law to attend school every day. The ultimate legal responsibility for school attendance (ARS 15-803) rests with parents and students, with the school assisting to fulfill that responsibility. Reasonable attempts will be made by the school through telephone calls and/or mail to inform parents/guardians of student absenteeism. Each student is allowed to accumulate five (5) absences during any one nine-week grading period. No more than 20 days in a school year. CMS will be tracking attendance very closely. Your child is expected to be at school every day unless there is an excused reason not to be. An absence is defined as a minimum of one missed class per period per day. An unexcused absence will count as a truant day as defined by law. A student is “habitually truant” if he/she has five or more unexcused absences from school or is absent more than ten percent or 18 days of the required number of school days per year whether the absence is excused or unexcused. Students absent for medical reasons, bereavement and/or prearranged special circumstances may be exempt from this requirement with proper documentation. A student will be allowed one day to make-up missed work for each day of absence. The regulation (J-131.1) and forms for attendance waivers are available in the school office.
Custody Issues: It is the responsibility of the parent to notify the school office in writing and maintain the proper documentation whenever a custody problem would change a parent’s ability to check a student out of school. CMS is a Closed Campus: Students will not be allowed to leave the school grounds for lunch or during school hours unless accompanied by a parent or guardian. Student Withdrawal: According to A.R.S 15-803, a student must be withdrawn from school records after ten (10) consecutive unexcused absences. If you need to withdraw your child from school, please come to the school office. The secretary will process the necessary paperwork for you. Visitors: All visitors must report to the office before entering any portion of the campus. Visitors are to sign in and wear a Visitor pass during their stay on campus. Student visitors are not allowed to attend class on our campus. Flower, Balloons and Present Deliveries: CMS will NOT accept deliveries of flowers, balloons and presents during school hours as it is an interference with the instructional day.
Absences: Call the CMS Office at 634-2231 to report your child’s absence. If no telephone contact has been made and the student fails to bring a note, the student will be marked with an unexcused absence and may be reported to legal authorities. Missing school without a valid excuse is truancy.
Emergency Plan Our District has worked with the Yavapai County Schools Office and adopted an Emergency Plan in accordance with the county and state guidelines. Administrators and staff have received varying levels of training which outline possible Emergencies and Procedures on how to safely and effectively respond to them. Students receive training and practice evacuations for different types of Emergencies including bus and fire on a regular basis.
Unexcused Absences: Students forfeit daily class grades for all unexcused absences. Disciplinary action can be assigned to students who are truant; i.e., detention (detainment after school hours) or In-School Alternative Program (ALC) placement. After 10 excused or 5 unexcused absences we may require an official doctor, dental, court, or funeral notice. After 4

Students are expected to follow the CMS guidelines about dress. There is considerable evidence to indicate a close relationship between student dress and student behavior. Students will not dress or groom themselves in a manner that, in the judgment of the school administrator, presents a risk to the health, safety or general welfare of the student, or other students or staff, or that is counterproductive to the district’s educational objectives.

Sunglasses, hairnets, bandanas, or ‘do-rags’ are not allowed.
•
Chains or spike jewelry of any kind are not allowed. Jewelry shall not display lewd, vulgar, obscene, plainly offensive language or symbols such as “I (heart) Boobies” bracelets.

Cosmetics that give the student a disturbing or distracting appearance are not allowed.
Any article of clothing, accessory, or cosmetics extreme enough to create a distraction, or disturb the normal routine of school, shall be deemed inappropriate. A final decision on the appropriateness of school dress will be determined by the administration. Continued failure to comply with the CMS dress code guidelines will be considered an insubordinate act and will be treated as such. The sound judgment of parents and students will most often avoid the problem with extreme costumes, fad dress, insufficient or inappropriate attire present in a wholesome school environment.

Hair color or style which draws undue attention and causes a disruption in the school shall not be permitted. (Designated Spirit Days are the exception.)

At CMS, we believe in logical consequences for actions. Students at CMS are encouraged to develop the habits of self-reliance and self-restraint.
Parents are required to sign the form at every level of the offense.
Each teacher will establish his/her limit setting for the classroom and discuss the expectations with the students during the first few days of school. It is in the student’s best interest that we work together, parent and staff, to support appropriate behavior in the classroom and on campus. Teachers and/or principal/assistant principal will be available to discuss questions you may have concerning student behavior.
Cell Phones Form New forms of harassment and bullying using cell phones have surfaced. Parents will be notified each time a student is in violation of this policy (JFCA). CMS has devised a step plan for cell phone and other electronic devices used inappropriately.
Bullying-Teasing-Threatening Form
1. A student violating the school cell phone policy will have his/her cell phone/electronic devices confiscated to be picked up from administration by a parent. 2. The second time the student will be assigned one (1) lunch detention in Alternative Learning Center (ALC) and the phone/device will need to be picked up from administration by a parent. 3. The third violation the student will be spend one (1) day in ALC and the phone/electronic device will need to be picked up from administration by a parent. 4. After the fourth visit to the office for violating school policy a meeting with a parent and administration will be held to develop a plan and determine the future consequences.
These offenses, including name-calling and harassing, can leave permanent emotional scars. Teasing/bullying/threatening has become serious ongoing problem at schools in the past few years. For the safety and well-being of our students, the following plan will be followed: 1. A student bullying/harassing another student verbally will be sent to the office for a first warning. 2. The 2nd offense will result in one (1) day in the ALC (Alternative Learning Center-Time Out) room. 3. The 3rd offense will result in two (2) days in the ALC room. 4. After the 4th offense, the student will be suspended from school. (Some types of teasing/bullying could merit off campus suspension.) Parents are required to sign the form at every level of the offense.

Dress Code Violation Form It is the belief of Cottonwood Middle School that students should dress appropriately for a learning environment. Continued non-compliance could result in suspension from school. CMS does have a form with four (4) steps explaining how the school deals with repeated dress code non-compliance on campus:

Continued use of inappropriate language may result in a parent, teacher, student, and Assistant Principal conference. Continued noncompliance could result in suspension from school. Public Displays of Affection (PDA) Public Displays of Affection (PDA) including hugging, holding hands, kissing, caressing, and other inappropriate actions as determined by CMS Administration is forbidden. At CMS, we do not allow PDA to take place on campus. This is in compliance with policy JFCA. Parents will be notified each time a student is in violation of this policy. Below you will find a plan that has been devised for public displays of affection. 1. A student in violation will be sent to the office for a first warning.
Severe Behavior includes: a. Student willfully inflicts physical harm on another student. b. Student willfully destroys property. c. Student overtly refuses to comply with a reasonable request.

Due Process Students in our schools have rights. They also have the responsibility to respect the rights and property of others. If a student fails to do this, disciplinary action will follow. In all situations, all students are entitled to due process. This means all students:

Depending upon the nature of the violation, student discipline shall be progressive, i.e., generally, a student’s first violation should merit a lighter penalty than subsequent violations. Engages in conduct that is disorderly, i.e., intentionally causing public inconvenience, annoyance, or alarm, or recklessly creating a risk thereof, by:

Secret Societies/GANG ACTIVITY (JICF-R) For the purpose of District policy, a gang is a group of three (3) or more people who: • • • • •
Interact together to the exclusion of others; Claim a territory or area; Have a name; Have rivals/enemies; and Exhibit antisocial behavior–often associated with crime or a threat to the community.
The type of dress, apparel, activities, acts, behavior or manner, or grooming displayed, reflected, or participated in by the student shall not: • Lead school officials to believe that such behavior, apparel, paraphernalia, activities, acts, hand signals, or other attributes are gang related or would disrupt or interfere with the school environment or activity and/or educational objectives; 8
• Present a physical safety hazard to self, students, staff member, or other employees; • Create an atmosphere in which the well-being of a student, staff member, or other person is hindered by undue pressure, behavior, intimidation, overt gesture, or threat of violence; or • Imply gang membership or affiliation by written communication, marks, drawing, painting, design,
or emblem upon any school or personal property or one’s person. If the student’s dress is in violation of this regulation or a District policy, the principal will ask the student to make the appropriate correction. If the student refuses, the parent/guardian may be notified and asked to make the necessary correction. The principal will take appropriate corrective and disciplinary action.
ELECTRONIC INFORMATION SERVICES ACCEPTABLE USER AGREEMENT
Details of the user agreement shall be discussed with each potential user of the electronic information services (EIS). Terms and Conditions / Acceptable use. Each user must: • Use the EIS to support personal educational objectives consistent with the educational goals and objectives of the School District. • Agree not to submit, publish, display, or retrieve any defamatory, inaccurate, abusive, obscene, profane, sexually oriented, threatening, racially offensive, or illegal material. • Abide by all copyright and trademark laws and regulations. • Not reveal home addresses, personal phone numbers or personally identifiable data unless authorized to do so by designated school authorities. • Take all responsible precautions, including password maintenance and file and directory protection measures, to prevent the use of personal and District accounts and files by unauthorized persons. • Understand that electronic mail or direct electronic communication is not private and may be read and monitored by school employed persons. • Trespassing in others' folders, work, or files is not acceptable. • Not use the networks in any way that would disrupt the use of the networks by others. • Not use the EIS for commercial purposes. • Use of personal devices during the school day is at the discretion of teachers and staff. Students must use devices as directed by their teacher for learning. • The primary purpose of the use of personal devices at school is educational. Personal use for personal reasons is secondary. • The use of a personal device is not to be a distraction in any way to teachers or students. Personal devices must not disrupt class in any way. • The use of personal devices falls under COCSD’s Acceptable Use Regulation, found in the student handbook. • Students will refrain from using personal devices outside of their classroom unless otherwise directed by their teacher. • Students shall not distribute pictures or video of students or staff without their permission (distribution can be as small as emailing/texting to one other person or as large as posting image or video online). 9
• Understand that inappropriate use may result in cancellation of permission to use the educational information services (EIS) and appropriate disciplinary action up to and including expulsion for students or termination for employees. Parent or Guardian Cosigner As the parent or guardian of the above named student, I have read the agreement in the District Parent/Student Handbook and understand it. I understand that it is impossible for the School District to restrict access to all controversial materials, and I will not hold the District responsible for materials acquired by use of the electronic information services (EIS). I also agree to report any misuse of the EIS to a School District administrator. (Misuse may come in many forms but can be viewed as any messages sent or received that indicate or suggest pornography, unethical or illegal solicitation, racism, sexism, inappropriate language, or other issues described in the agreement.) I accept full responsibility for supervision if, and when, my child's use of the EIS is not in a school setting. If the student user is under eighteen years of age, the parent/guardian of the student must agree to accept all financial and legal liabilities that may result from that student's use of the Cottonwood-‐Oak Creek School District’s networks. Young Adult Permission Slip
The library at Cottonwood Middle School has put markers on books that deal with mature subject matter or have mature language. YA books are labeled for the mature reader. Many of these books have been recognized as outstanding literature for sensitive topics or issues that youth deal with during their teenage years. Only 7th & 8th grade students will be eligible to check out YA books. CMS administration, along with the Librarian suggests that you preview the YA book your child has checked out to determine if you are comfortable with the topic and language. I have read and understand that Cottonwood-Oak Creek School District does not carry medical or accident insurance for students, but insurance can be obtained by visiting the district/school’s website at www.cocsd.k12.az.us, or pick up a brochure at the school office.
