Medical Marijuana Registry Steps to request access to your patient registration If you aren’t able to create a registration using your information, follow the steps below to request access to your patient registration online.

1. Login to your account.

2.Click “New Request Access to Patient Registration.” This will open a new form.

3. Select your application type.

4. Enter your information.

5. Click “browse” to upload a copy of your valid Colorado ID or driver’s license.

6. Enter your name to sign the form.

7. Click “save” to submit your form.

8. After you submit your form, your request will be pending.

Registry staff will review your information. You will receive an email once your registration has been processed.

9. Once your request is approved, log in and view your patient registration. Click on your name to view your patient registration.
