Dear Parents/Guardians: Each year, we request your preference and attention on a few key issues. We only ask for your preference on these issues at the beginning of the school year. Please fill out your child’s name above. Your signature is required at the end.

1. Release of Student Directory Data - State law and board policy protects the confidentiality of student records. Directory information shall not be provided to any organization for profit-making purposes nor for the purpose of marketing or for selling that information. An exception has been made, however, for student information considered to be directory data. Unless you decline release of this directory data (below), directory data becomes a public record. The Oshkosh Area School District defines high school directory data as student’s name, date and place of birth, dates of attendance, photographs, honors, and the name of the school most recently attended by the student.  Yes, you may release my student’s directory data.  Yes, but you may only release my student’s directory data for yearbook purposes.  No, please do not release my child’s directory data (This includes for yearbook purposes).

2. Student Directory Data Provided to Military and Institutions of Higher Learning (No Child Left Behind Act of 2001 and Board Policy)—As part of this legislation, local school districts receiving federal assistance are required to provide, when requested by military recruiters or institutions of higher education, access to secondary school students’ names, addresses and phone numbers. A secondary school student, or the parent of student, may request that the student’s information not be shared. You may share personal information (name, address, phone number) about my child with:

3. Permission for Students to be Interviewed, Photographed or Videotaped in School - Students may be photographed or videoed for educational purposes. Furthermore, these images/videos may be accessible via the Internet for school learning activities and projects. TV, radio, newspapers, or other entities may request to interview, photograph or video students. Parental consent is not required when photos/videos are of large groups or when they are available only to school district staff/students.  Yes, my child may be photographed, videotaped or interviewed if the principal or my child’s teacher believes it to be appropriate  No, my child may not be photographed, videotaped or interviewed by the media or other entities 4. OASD “Learning without Limits” Chromebook Program Parent/Guardian Agreement As the parent or legal guardian, I have read and understand that my child or ward shall comply with the terms of the Oshkosh Area School District's “Learning without Limits” Student/Parent Handbook. I understand that the chromebooks are a privilege and can be revoked if misused. I understand that if the chromebook is damaged, lost, or stolen that my child or myself will be responsible to reimburse the District for the cost of the repair or replacement as set forth in the handbook. Further, I accept full responsibility for supervision of my child's or ward's use of his or her chromebook if and when such access is not in the school setting. I have read the OASD “Learning without Limits” Chromebook Program Parent/Guardian Agreement and indicated my preferences above regarding my child for the 2016-2017 school year.
