The lnformation and Communications Technology Service (lCfS) is the office in-charge of carrying-out
the rollout of the oepEd Computerization Program (DCP). Recently, during the deployment of DCP packages under the ocP 2015 budget (batch 33), Suppliers were Biven a softcopy of the lnspection and Acceptance Report (lAR) which were provided to each recipient school at the time the DCP packages were delivered.
l..lnfortunately, during consolidation of Acceptance documents (i.e. Delivery Receipts, Training Checklists and lARs) from the aforementioned batch which were accomplished by school officials/personnel and submitted by Suppliers for processing, it was found out that there was a discrepancy between the Delivery receipt and the lAR. The IAR did not specify one (1) item that was included in the package but was present in the Delivery receipt. That item is an external portable DVD Writer. Last lanuary 2017, a memorandum (copy attached) was sent

to all Division lnformation
Technology Officers (lTO) asking for assastance in obtaining another IAR (si8ned by authorized school officials/personnel) indicating only the external portable OVD Writer and forward the consolidated lARs to lcTs - oepEd Central Office. Sadly, lcTs found out that due to the large number and Sreat distance of schools to be covered, the task would take a long time to accomplish.
ln this regard, we ask for your assistance in expediting a Certiflcate of Receipt and Acceptarce (template attached) which will replace the previously desired lARs that the Division lTOs need to gather from each recipient school. with School lD (per division); lndication of number of Batch 33 packages received per recipient school; lndication of Supplier of the Batch 33 DCP package for the recipient school, for example: a. Advance Solutions, lnc. (A5l) b. Columbia Technologies, lnc. (CTl) c. Red Dot lmaging Philippines, lnc. List of Batch 33 recipient schools
